Clinical application value of new ultra-fine transnasal gastroscope combined with pigment staining for outpatient opportunistic screening of early gastric cancer and precancerous lesions

Objective To explore the clinical application value of new ultra-fine transnasal gastroscope combined with pigment staining for outpatient opportunistic screening of early gastric cancer and precancerous lesions.Methods From January 2016 to May 2018,the latest Japanese Fujifilm EG-530 NW ultra-fine transnasal gastroscope was inserted through the nasal cavity to examine the upper digestive tract of 7421 outpatients in the endoscopy room of our hospital.The pathological results of biopsy in patients were counted,and the clinical and pathological features and postoperative pathological results of patients with precancerous lesions,early gastric cancer and advanced gastric cancer were analyzed;the results of biopsy and postoperative pathology in patients with early gastric cancer and precancerous lesions were analyzed.Results Among the 7421 patients,164 cases(2.21%)had mild to moderate dysplasia or low-grade intraepithelial neoplasia,and 154 cases(2.08%)had gastric cancer,including 51 cases(33.12%)with early gastric cancer/high-grade intraepithelial neoplasia and 103 cases(66.88%)with advanced gastric cancer.Conclusion The new ultra-fine transnasal gastroscope can be used as one of the effective methods for screening early gastric cancer and precancerous lesions of upper digestive tract in primary medical units because of its soft tube,good patient tolerance,high endoscopic follow-up compliance and simplicity for primary digestive endoscopists to master this examination technique.
