针灸治疗腰椎间盘突出症的临床效果及远期神经肌电图变化情况

目的探讨针灸治疗腰椎间盘突出症的临床效果及远期神经肌电图变化情况。方法选择我院2015年1月至2017年1月收诊的120例腰椎间盘突出症患者为研究对象,按照随机数字表法将其分为对照组和观察组,各60例。对照组采取常规治疗(腰椎牵引),观察组在腰椎牵引的基础上实施针灸治疗。比较两组的治疗效果。结果观察组的近期治疗总有效率高于对照组(P<0.05)。治疗后,两组患者的CRP、IL-6水平及腰椎疼痛评分均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的腰椎功能评分、腰椎活动度及生活质量评分均较治疗前升高,且观察组高于对照组(P<0.05)。随访3年时,两组的胫神经F波潜伏期及腓神经F波潜伏期均较治疗前缩短,胫神经F波传导速度及腓神经F波传导速度均较治疗前升高,且观察组优于对照组(P<0.05)。结论采用针灸治疗腰椎间盘突出症患者可获得良好的近、远期效果,且能降低腰椎疼痛度及炎性因子水平,改善患者的腰椎功能及生活质量。

Clinical effect of acupuncture and moxibustion on lumbar disc herniation and the long-term changes of neuroelectromyogram

Objective To explore the clinical effect of acupuncture and moxibustion on lumbar disc herniation and the long-term changes of neuroelectromyogram.Methods One hundred and twenty patients with lumbar disc herniation treated in our hospital from January 2015 to January 2017 were selected as the study objects and divided into control group and observation group according to random number table method,with 60 cases in each group.The control group was given conventional treatment(lumbar traction),and the observation group was treated with acupuncture and moxibustion on the basis of lumbar traction.The therapeutic effects of the two groups were compared.Results The short-term total effective rate of treatment in the observation group was higher than that of the control group(P<0.05).After treatment,the levels of CRP,IL-6 and lumbar pain score of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the lumbar function scores,lumbar range of motion and quality of life scores in the two groups were higher than those before treatment,and those in the observation group were higher than the control group(P<0.05).At 3 years of follow-up,the tibial nerve F-wave latency and peroneal nerve F-wave latency of the two groups were shorter than those before treatment,and the tibial nerve F-wave conduction velocity and peroneal nerve F-wave conduction velocity were higher than those before treatment,and those of the observation group were better than the control group(P<0.05).Conclusion Acupuncture and moxibustion on patients with lumbar disc herniation can obtain good short-term and long-term effects,reduce the lumbar pain degree and inflammatory factor levels,and improve the lumbar function and quality of life of patients.
