后腹腔镜微创取石术治疗输尿管上段结石合并轻度肾积水的临床效果

目的探讨后腹腔镜微创取石术治疗输尿管上段结石合并轻度肾积水的临床效果。方法选取本院收治的100例输尿管上段结石合并轻度肾积水患者,依据手术方法将其分为后腹腔镜微创取石术组(n=50)和输尿管镜碎石取石术组(n=50)。比较两组的治疗效果。结果后腹腔镜微创取石术组的术中出血量少于输尿管镜碎石取石术组,手术时间长于输尿管镜碎石取石术组,结石清除率高于输尿管镜碎石取石术组,术后并发症总发生率低于输尿管镜碎石取石术组(P<0.05)。手术后,两组的血清TNF-α、IL-6、KIM-1、Cys-C水平均升高,IL-2水平均降低,但后腹腔镜微创取石术组均优于输尿管镜碎石取石术组(P<0.05)。结论后腹腔镜微创取石术治疗输尿管上段结石合并轻度肾积水,可减少术中出血量和并发症,减轻机体炎症反应,提高结石清除率。

Clinical effect of retroperitoneal laparoscopic minimally invasive lithotomy in the treatment of upper ureteral calculi with mild hydronephrosis

Objective To investigate the clinical effect of retroperitoneal laparoscopic minimally invasive lithotomy in the treatment of upper ureteral calculi with mild hydronephrosis.Methods A total of 100 patients with upper ureteral calculi and mild hydronephrosis admitted in our hospital were selected and divided into retroperitoneal laparoscopic minimally invasive lithotomy group(n=50)and ureteroscopic lithotripsy group(n=50)according to the surgical method.The therapeutic effects of the two groups were compared.Results The intraoperative blood loss volume of the retroperitoneal laparoscopic minimally invasive lithotomy group was less than that of the ureteroscopic lithotripsy group,the operation time was longer than that of the ureteroscopic lithotripsy group,the stone clearance rate was higher than that of the ureteroscopic lithotripsy group,and the total incidence in postoperative complications was lower than that of the ureteroscopic lithotripsy group(P<0.05).After operation,the serum levels of TNF-α,IL-6,KIM-1 and Cys-C in the two groups increased,while the levels of IL-2 decreased,but those in the retroperitoneal laparoscopic minimally invasive lithotomy group were better than the ureteroscopic lithotripsy group(P<0.05).Conclusion Retroperitoneal laparoscopic minimally invasive lithotomy for upper ureteral calculi with mild hydronephrosis can decrease intraoperative blood loss volume and complications,reduce the body's inflammatory response,and improve the stone clearance rate.
