缬沙坦联合氨氯地平治疗老年高血压的临床效果

目的分析缬沙坦联合氨氯地平治疗老年高血压的临床效果。方法选取2018年10月至2019年9月于我院接受治疗的180例老年高血压患者,根据随机数字表法将其分为对照组和观察组,各90例。对照组采用缬沙坦+氢氯噻嗪治疗,观察组采用缬沙坦+氨氯地平治疗,两组均持续治疗8周。比较两组患者治疗前、后的血压变异性、血管弹性、生活质量评分及不良反应发生情况。结果治疗前,两组昼、夜SSD、DSD无显著差异(P>0.05);治疗后,两组昼、夜的SSD、DSD均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组踝臂指数、PWV、C2、C1无显著差异(P>0.05);治疗后,观察组的踝臂指数、PWV、C2、C1均优于对照组及治疗前(P<0.05)。治疗前,两组生活质量各维度评分无显著差异(P>0.05);治疗后,两组生理领域、心理领域、社会领域评分均高于治疗前,且观察组高于对照组,差异具有统计学意义(P<0.05)。观察组的不良反应总发生率低于对照组,差异具有统计学意义(P<0.05)。结论缬沙坦联合氨氯地平治疗老年高血压能有效降低患者血压变异性,维持血压平稳,促进血管弹性恢复,提高患者的生活质量,且不良反应较少。

Clinical effect of valsartan combined with amlodipine in the treatment of elderly hypertension

Objective To analyze the clinical effect of valsartan combined with amlodipine in the treatment of elderly hypertension.Methods A total of 180 elderly patients with hypertension treated in our hospital from October 2018 to September 2019 were selected and divided into control group and observation group according to the random number table method,with 90 cases in each group.The control group was treated with valsartan and hydrochlorothiazide,and the observation group was treated with valsartan and amlodipine.Both groups were treated for 8 weeks.The blood pressure variability,vascular elasticity,quality of life score before and after treatment and adverse reactions of the two groups were compared.Results Before treatment,there were no significant differences in SSD and DSD at daytime and night between the two groups(P>0.05);after treatment,the SSD and DSD at daytime and night in the two groups were lower than those before treatment,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in ankle brachial index,PWV,C2 and C1 between the two groups(P>0.05);after treatment,ankle brachial index,PWV,C2 and C1 of the observation group were better than those of the control group and before treatment(P<0.05).Before treatment,there were no significant differences in the scores of each dimensions of quality of life between the two groups(P>0.05);after treatment,the scores of physiological field,psychological field and social field of the two groups were higher than those before treatment,and those in the observation group were higher than the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Valsartan combined with amlodipine in the treatment of elderly hypertension can effectively reduce blood pressure variability,maintain stable blood pressure,promote the recovery of vascular elasticity,and improve the quality of life of patients,with less adverse reactions.
