体外膜肺氧合技术在9例危重症患者中的应用效果

摘    要:目的总结重症医学科医护团队主导的体外膜肺氧合(ECMO)在危重症患者中的应用效果。方法收集和分析9例经ECMO治疗患者的基本信息、ECMO相关参数、治疗、转归、动脉血气指标及并发症等资料。结果9例患者中,7例成功撤机,5例存活出院,4例死亡。死亡患者中1例并发大面积脑出血,1例消化道大出血,1例感染性休克,1例放弃治疗。ECMO运转时间为26~384 h。患者治疗前及治疗后1、3、7 d的PaO2、Lac水平整体比较,差异具有统计学意义(P<0.05)。结论ECMO治疗能迅速改善患者的血流动力学稳定性,提高心脏骤停患者的抢救成功率。

Abstract:Objective To summarize the application effect of extracorporeal membrane oxygenation(ECMO)led by the medical team of critical care department in critically ill patients.Methods The basic information,ECMO related parameters,treatment,outcome,arterial blood gas indexes and complications of 9 patients treated with ECMO were collected and analyzed.Results Of the 9 patients,7 cases were successfully withdrawn,5 cases survived and discharged,and 4 cases died.Among the dead patients,1 case was complicated with massive cerebral hemorrhage,1 case was gastrointestinal hemorrhage,1 case was septic shock and 1 case gave up treatment.The operation time of ECMO was 26-384 h.The overall comparison of PaO2 and Lac levels before treatment and at 1,3 and 7 days after treatment showed significant differences(P<0.05).Conclusion ECMO treatment can rapidly improve the hemodynamic stability of patients and improve the success rate of rescue in patients with cardiac arrest.
