伴缺血性脑卒中史急性冠脉综合征患者PCI术后疗效的影响因素分析

目的分析伴缺血性脑卒中史急性冠脉综合征(ACS)患者经皮冠状动脉介入(PCI)术后疗效的影响因素。方法选取我院2016年10月至2018年10月收治的伴缺血性脑卒中史的ACS患者78例,均接受PCI术治疗。术后随访1年,根据随访情况将研究对象分为疗效良好组和疗效不佳组,分析影响患者PCI术后疗效的相关因素。结果门诊随访1年,78例伴缺血性脑卒中史ACS患者中,19例发生心脏不良事件,发生率为24.36%。两组的年龄、糖尿病病史、高脂血症病史、吸烟史、高血压病史比较,差异具有统计学意义(P<0.05);疗效不佳组的hs-CRP、Fbg、TG、HbA1c水平高于疗效良好组(P<0.05)。Logistic回归分析结果显示,年龄、糖尿病病史、吸烟史、高血压病史、hs-CRP、Fbg、HbA1c水平为伴缺血性脑卒中史ACS患者PCI术后疗效的影响因素(P<0.05)。结论年龄、糖尿病病史、吸烟史、高血压病史及hs-CRP、Fbg、HbA1c水平为伴缺血性脑卒中史ACS患者PCI术后疗效的影响因素,临床应尽早明确上述影响因素,以制定针对性干预措施,确保患者PCI术后治疗效果。

Analysis of the influencing factors on PCI efficacy of acute coronary syndrome patients with ischemic stroke history

Objective To analyze the influencing factors on percutaneous coronary intervention(PCI)efficacy of acute coronary syndrome(ACS)patients with ischemic stroke history.Methods A total of 78 ACS patients with ischemic stroke history admitted in our hospital from October 2016 to October 2018 were selected and received PCI.The patients were followed up for 1 year.According to the follow-up,the subjects were divided into good efficacy group and poor efficacy group,and the related factors affecting the PCI efficacy of patients were analyzed.Results After 1 year of the outpatient clinic follow-up,19 cases had adverse cardiac events among 78 ACS patients with ischemic stroke history,and the incidence rate was 24.36%.There were significant differences in age,history of diabetes,hyperlipidemia,smoking and hypertension between the two groups(P<0.05);the levels of hs-CRP,Fbg,TG and HbA1c in the poor efficacy group were higher than those in the good efficacy group(P<0.05).Logistic regression analysis showed that age,history of diabetes,smoking and hypertension,hs-CRP,Fbg and HbA1c levels were the influencing factors of the PCI curative effect of ACS patients with ischemic stroke history(P<0.05).Conclusion Age,history of diabetes,smoking,hypertension,hs-CRP,Fbg,HbA1c levels are the influencing factors of the PCI curative effect of ACS patients with ischemic stroke history.The above influencing factors should be identified as soon as possible in order to formulate targeted intervention measures to ensure the PCI curative effect of ACS patients.
