妊娠晚期组织学绒毛膜羊膜炎与妊娠结局的关系探讨

目的探讨妊娠晚期组织学绒毛膜羊膜炎与妊娠结局的关系。方法选取40例分娩时确诊为组织学绒毛膜羊膜炎产妇作为观察组,选取40例同期正常分娩产妇为对照组。比较两组妊娠结局、产后出血、产褥感染、胎盘胎膜残留情况以及新生儿结局。结果观察组孕妇剖宫产率72.5%、胎儿窘迫率37.5%、早产率52.5%、产褥感染率10.0%均高于对照组的50.0%、17.5%、17.5%、0,差异均具有统计学意义(P<0.05)。两组胎膜早破率、死胎率、产后出血率、胎盘胎膜残留率比较,差异均无统计学意义(P>0.05)。观察组低出生体重儿、新生儿感染、病理性黄疸发生率分别为52.5%、47.5%、75.0%,均高于对照组的17.5%、22.5%、52.5%,差异有统计学意义(P<0.05)。结论妊娠晚期组织学绒毛膜羊膜炎与妊娠结局具有密切关系,加强组织学绒毛膜羊膜炎早期预测和干预,对于提高妊娠结局质量、保证母婴安全具有重要意义。

The correlation between histologic chorioamnionitis in third trimester of pregnancy and pregnancy outcome

Objective To discuss the correlation between histologic chorioamnionitis in third trimester of pregnancy and pregnancy outcome.Methods There were 40 cases of parturients with histological chorioamnionitis selected as the observation group,and 40 cases of parturients with normal delivery as the control group.The pregnancy outcomes,postpartum hemorrhage,puerperal infection,residue of placental membranes,and neonatal outcomes between the two groups were compared between the two groups.Results The cesarean section rate 72.5%,fetal distress rate 37.5%,preterm birth rate 52.5%,and puerperal infection rate 10.0%of the observation group were all higher than 50.0%,17.5%,17.5%and 0 of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of premature rupture of membranes,stillbirth rate,postpartum hemorrhage rate,residual rate of placental membranes between the two groups(P>0.05).The incidence of low birth weight infants,neonatal infections,pathological jaundice of the observation group were 52.5%,47.5%and 75.0%,which were all higher than 17.5%,22.5%and 52.5%of the control group,and the difference was statistically significant(P<0.05).Conclusion Histological chorioamnionitis in the third trimester of pregnancy is closely related to pregnancy outcome.Strengthening the early prediction and intervention of histological chorioamnionitis is of great significance for improving the pregnancy outcome and ensuring the safety of mothers and babies.
