老年冠心病患者血清瘦素及可溶性瘦素受体检测分析

摘    要:目的 研究老年冠心病患者血清瘦素及可溶性瘦素受体水平变化及影响因素。方法 应用放射免疫分析法 (RIA)及酶联免疫吸附分析法 (ELISA)检测 34例老年冠心病患者及其 36例对照者血清瘦素 (Lp)、可溶性瘦素受体 (sLR)、空腹血糖 (FBG)、总胆固醇 (TC)、甘油三酯 (TG)、高密度脂蛋白 (HDL)、低密度脂蛋白 (LDL)、胰岛素 (Ins)、HOMA胰岛素抵抗指数 (HOMA IR)、体质量指数 (BMI)、腰围、臀围、腰臀比 (WHR)等临床指标 ,分析Lp、sLR与血脂、胰岛素抵抗及冠心病的关系。结果 老年冠心病组Lp、Ins及HOMA IR水平明显高于对照组 ,sLR水平明显低于对照组 (P <0 .0 5或 0 .0 0 1)。Lp与BMI、腰围、臀围、Ins、HOMA IR、TC、TG呈正相关 ,与sLR呈负相关 ;sLR与Lp、BMI、腰围、臀围呈负相关 (P <0 .0 5或 0 .0 1)。结论 瘦素及瘦素受体水平异常可能与冠心病的发病密切相关。

Abstract:Objective To investigate the serum leptin (Lp) and soluble leptin receptor (sLR) levels in elderly patients with coronary heart disease(CHD). Methods Lp, sLR, TC, TG, HDL, LDL, FBG,Ins, insulin resistance(IR)(HOMA-IR), body mass index(BMI), waist and hip circumferences,waist to hip ratio(WHR) were determined in 36 health persons as control group and 34 elderly patients with coronary heart disease. Serum Lp levels were measured by radioimmunoassay(RIA) and sLR levels were measured by enzyme-linked immunosorbent assay(ELISA). To analyze the relationship of Lp/sLR to dyslipidemia, IR and coronary heart disease. Results The patients had significantly higher Lp,Ins, HOMA-IR but lower sLR than that of the control group(P< 0.05 or P< 0.001). Lp was in positive relationship with Ins, HOMA-IR, TC,TG, BMI,waist and hip circumferences, but in negative relationship with sLR(P< 0.05 or P< 0.01). sLR was in negative relationship with Lp, BMI, waist and hip circumferences (P< 0.05 or P< 0.01). Conclusion These results suggest abnormal levels of Lp and sLR might be involved in the pathogenesis of CHD.
