西沙必利在肠镜检查肠道准备中的作用

目的：探讨提高结肠镜检查肠道准备效果的方法。方法：观察组检查前3d服西沙必利5mg，3次／d，检查前晚服50％硫酸镁溶液90ml，对照组不服西沙必利，肠道清洁度差者，开塞露2支注入直肠，排便后作结肠镜。结果：观察组205例，对照组206例，清洁者55．1％和20．9％，良好28．3％和19．4％，尚可13．7％和34．9％，差2．9和24．8％，插镜达盲肠或回肠末端者70．7％和58．7％，32例使用开塞露者，清洁65．5％，良好28．1％，结论：西沙必利对结肠镜检查肠准备有良好的促进作用，开塞露是肠道清洁度差者理想的补救方法。

Efficacy of Cisapride in Bowel Preparation for Colonoscopy

Objective: To investigate the methods of improving effect of bowel preparation for colonoscopy. Methods: The patients were randomly divided into two groups (A and B). All patients took low residue diet for two days, drank 90ml of 50% magnesium sulfate solution and then some 3000ml water in evening of the day before colonoscopy, but group A received 5mg of cisapride three time a day for three days before colonoscopy. Endoscopists were blinded to the assignment way of bowel preparation. The cleaning grade of bowel preparation was established as excellent, good, fair and poor. 40 ml of 50% glycerin solution were injected into rectum of the patients with poor cleaning grades, after they had excreted stool colonoscopy again was done. Results: Group A and B were 205 and 206 patients respectively. In A and B, the cleaning grade of bowel preparation were : excellent 55.1% and 20.9% (P<0.005), good 28.3% and 19.4% (P<0.05), fair 13.7% and 34.9% (P<0.005), poor 2.9% and 24.8% (P<0.005) respectively. The patients whose cecum and terminal ileum was reached were 70.7% and 58.7% (P<0.05). Side effect were: abdominal pain 4.4% and 0.9% (P<0.05), bloating 1.0% and 9.2% (P<0.005) respectively. In 32 patients having used glycerin, excellent grade was 65.6 %, good 28.1%, fair 6.3%. Conclusions: Cisapride has better effect on bowel preparation for colonoscopy. Introducing 40ml of 50% glycerin solution into rectum is an ideal remedy to the patients with poor colon cleaning grade for colonoscopy.
