强化教育对慢性乙型肝炎患者口服核苷(酸)类似物治疗依从性的影响

摘    要:目的研究强化教育对提高慢性乙型肝炎患者口服核苷(酸)类似物抗病毒治疗依从性的作用。方法将198例服用核苷(酸)类似物抗病毒治疗的慢性乙型肝炎患者随机分为强化教育组(100例)和对照组(98例)。第一次处方药物时,对所有患者均进行关于口服抗病毒治疗依从性的教育,在上述基础上,对强化教育组患者每2周左右通过电话、手机短信或电子邮件等方式进行口服抗病毒治疗依从性的强化教育。在4、12、24和48周时,对两组患者的治疗依从性进行比较。结果在4、12、24和48周内,强化教育组患者的自行停药率分别为0、1.0%、3.0%以及3.0%,连续服药(无漏服)率分别为98.0%、97.0%、97.0%和96.0%;相应的时间点内,对照组患者的自行停药率分别为1.0%、3.1%、7.1%和9.2%,连续服药率分别为96.9%、94.9%、91.8%和88.8%,强化教育组患者24、48周时的治疗依从性优于对照组患者。结论对口服核苷(酸)类似物抗病毒治疗的慢性乙型肝炎患者进行强化教育,有助于提高他们的治疗依从性。

Abstract:Objective To study the role of enhanced education in improving the compliance of patients with chronic hepatitis B for taking nucleotide analogues.Methods One hundred and ninety-eight patients with chronic hepatitis B taking nucleotide analogues were randomly allocated into enhanced education group(n=100) and control group(n=98).Essential information about the importance of compliance was given to all patients when they were prescribed with nucleotide analogues for the first time.Besides above measures,patients in enhanced health education group were urged and encouraged additionally by call and/or information through mobile telephones or email every 2 weeks for emphasizing the importance of compliance for taking nucleotide analogues.At 4th,12th,24th and 48th week,patients in both groups were evaluated in term of compliance for taking nucleotide analogues.Results At 4th,12th,24th and 48th week,the improper drug discontinuance rates were 0,1.0%,3.0% and 3.0% respectively in patients of enhanced education group,in comparison with 1.0%,3.1%,7.1% and 9.2% in patients of control group.There were 98.0%,97.0%,97.0% and 96.0% of patients keeping continuous therapy without interruption during 4th,12th,24th and 48th week of treatment in enhanced education group in comparison with 96.9%,94.9%,91.8% and 88.8% of those in control group.The compliance of patients in 24th and 48th week was significantly improved by enhanced education.Conclusion Enhanced education can improve the compliance of patients with chronic hepatitis B for taking nucleotide analogues.
