射频消融治疗胰腺肿瘤安全性的临床研究

目的:探讨射频消融治疗胰腺肿瘤的安全性。方法:回顾性分析2002年1月-2007年12月对78例不能切除胰腺肿瘤的患者行射频消融治疗发生的并发症资料。结果:所有患者均有短期发热症状,部分患者有一过性血清淀粉酶升高,无腹腔出血及腹腔感染发生,胰瘘2例,上消化道出血1例,血管并发症2例,急性重症胰腺炎1例,术后总并发症发生率为7.69%(6/78),其中死亡3例,发生率为3.85%(3/78)。结论:对于不能切除的胰腺肿瘤射频治疗是安全可行的,但由于可能出现致命性严重并发症,必须慎重。

Clinical study on safety of radiofrequency ablation for pancreatic tumors

Objective: To evaluate the safety of radiofrequency ablation(RFA) for unresectable pancreatic tumors.Methods: During January 2002 to December 2007,78 patients with unresectable pancreatic tumor were treated by open cool-tip RFA.Clinical records of complications were retrospectively studied.Results: Temporary fever occurred in all patients,and transient elevated blood anylase occurred in part of patients,and no celiac bleeding and no celiac abscess.The incidence of post-operation complications was 7.69%(6/78),and the mortality was 3.85%(3/78).In the 6 cases,two were pancreatic leakage,one was upper gastrointestinal bleeding,one was acute severe pancreatitis,and the other two were vassular complications.Conclusion: Radiofrequency ablation for unresectable pancreatic tumors is generally safe and feasible.However,it may cause severe and life-threatening complications.The management of RFA must be prudent and careful.
