无创辅助通气抢救肺性脑病的疗效观察

摘    要:目的探讨无创辅助通气抢救肺性脑病患者的临床效果。方法2011—01~2012—12收治的COPD合并肺性脑病患者120例,随机分为3组,三组患者均给予抗炎、平喘、祛痰、扩张支气管等基础治疗,无创通气组在此基础上应用无创通气治疗,有创通气组除基础治疗外应用有创辅助通气,三组患者于治疗前后进行血气分析,心率及观察治疗前后,晦床症状改变,对相关数据进行统计分析。结果3组患者治疗前血气分析结果无明显差异(P〉0.05),通过治疗后,3组患者血气分析结果均有所改变,但有创通气组与无创通气组改善较基础治疗组明显,PH、PaO2、SaO2均明显升高,PaCO2明显降低,差异有统计学意义(P〈0.05)。3组住院时间比较,无创通气组患者平均住院时间明显低于其余两组,差异有统计学意义,其中P〈0.05,而基础治疗组与有创通气组住院时间无明显差异。结论无创辅助通气用于COPD合并肺性脑病的患者可显著提高患者PH、PaO2、SaO2,有效,降低PaCO2,住院时间短,是临床切实可行的方法。

Noninvasive Assisted Ventilation Rescue Curative Effect Observation of Pulmonary Encephalopathy

Abstract:Objective To study the noninvasive assisted ventilation rescue the clinical effect of patients with pulmonary encephalopathy. Methods The 2011 - 01 - 2012 - 12 treated 120 cases of patients with COPD with pulmonary encepha- lopathy, randomly divided into 3 groups, three groups of patients were given anti - inflammatory, bronchial asthma, ex- pectorant, expansion, foundation treatment, such as noninvasive ventilation group on the basis of the treatment, the ap- plication of noninvasive ventilation and ventilation group in addition to the foundation treatment application has assisted ventilation, blood gas analysis in three groups of patients before and after treatment, heart rate and changes were ob- served before and after treatment of clinical symptoms, the related data for statistical analysis. Results The three groups of patients before blood gas analysis results no significant difference ( P 〉 0.05 ), after treatment, 3 patients blood gas a- nalysis results are changed, but improved the ventilation and noninvasive ventilation group obviously compared with the base, PH, PaO2, SaO2 were significantly elevated, PaCO2 decrease, the difference was statistically significant ( P 〈 0. 05 ). Three groups of length of hospital stay, average hospital stay in patients with nnninvasive ventilation group was obviously lower than the rest of the two groups, difference was statistically significant, among them (P 〈 0. 05, and the basis of the treatment group and invasive ventilation group no difference between the length of hospital stay. Conclusions noninvasive assisted ventilation for COPD patients can significantly improve patients with pulmonary encephalopathy PH, PaO2, SaO2, effectively, reduce PaCO2, shorter hospitalization time, is practical and feasible method.
