不同子宫肌瘤类型合并妊娠分娩方式与手术处理的探讨

目的：探讨不同子宫肌瘤类型合并妊娠分娩方式与手术处理的方法，提高手术治疗效果。方法：回顾性分析我院自2006年3月~2010年5月收治的60例妊娠合并子宫肌瘤患者的临床资料，按照不同子宫肌瘤类型对手术情况、产后出血量、产妇预后等方面进行探讨。结果：（1）合并浆膜下肌瘤组阴道分娩方式较剖宫产术中出血量少，但无统计学意义（P〉0.05），合并肌壁间肌瘤、多发性肌瘤组阴道分娩方式均较剖宫产术中出血量多（P0.05）；合并肌壁间肌瘤组两种分娩方式比较差异显著（t=4.514，P0.05），合并肌壁间肌瘤组两种分娩方式并发症发生率比较差异显著（x2=7.264， P0.05），剖宫产联合子宫肌瘤剔除术与前两者比较差异显著（x2=16.754，P〈0.01；x2=15.274， P〈0.01）。结论：根据不同子宫肌瘤类型选择不同分娩方式可降低手术创伤、术后出血量及并发症，有利于产妇康复及子宫肌瘤治愈。

Different types of uterine fibroids merge the pregnancy childbirth way and surgical treatment

Objective：To investigate different types of uterine fibroids with pregnancy and surgical delivery mode processing methods to improve treatment outcome. Results： （1） combined subserosal myoma group compared with cesarean section vaginal delivery means less blood loss, but no statistical significance （P〉 0.05）, combined intramural fibroids, multiple fibroids vaginal delivery group way more blood loss than cesarean section （P0.05）;merger of two intramural myoma group was significant difference between mode of delivery （t=4.514, P0.05）, intramural myoma combined mode of delivery complications in the two groups was significant difference between the incidence of （x2 = 7.264, P 0.05）, cesarean myomectomy combined with a significant difference between the two （x2=16.754, P〈0.01;x2=15.274 , P〈0.01）. Conclusion：According to different types of uterine fibroids choose a different mode of delivery can reduce the surgical trauma and postoperative bleeding and complications, rehabilitation and maternity benefit to cure uterine fibroids.
