慢性特发性便秘肛门直肠动力学研究

摘    要:目的:探讨慢性特发性便秘患者的肛门直肠动力学因素的变化.方法:采用PC PolygrafHR高分辨多道胃肠功能测定仪检测20例患者,15名健康者作对照.结果:与正常对照组相比,慢性特发性便秘组患者肛门内、外括约肌功能异常,直肠感觉功能损害,肛门括约肌协同失调.结论:慢性特发性便秘患者存在肛门直肠动力学的异常变化,且受多因素影响.

Abstract:Objective To study the correlating factors of anorectal dynamic changes in patients with chronic idiopathic constipation (CIC) Methods Multichannel and high-resolution anus rectum function detector of PC Polygraph HR was adopted in 20 CIC patients and 15 healthy persons (as control). Results Three anorectal dynamic abnormalities in CIC patients were detected:(1)dysfunction of the anal internal sphincter and external sphincter in terms of reduced anorectal sphincteric resting pressure and maximum squeezing pressure(P<0.05),increased minimum relaxation volume to elicit the anorectal inhibitory reflex(MRV,P<0.005) and decreased relaxation rate of the anal sphincter(P<0.025);(2) functional damage of rectal perception ,as shown by increase of both the defecatory threshold caused by rectal volume (P<0.005) and the rectal maximum tolerable volume (P<0.005) and (3) dyssynergia of the anal sphincter,as shown in the experiment of simulated defecation action and viewed defectory pressure curves,86% CIC patients showed abnormal B and C curves,while the controls all showed normal A curves Conclusion Patients with CIC have abnormalities in anorectal dynamics.
