支气管哮喘患儿痰液嗜酸粒细胞及其阳离子蛋白水平的动态变化

摘    要:摘要 目的 探讨不同病理生理状态支气管哮喘患儿痰液嗜酸粒细胞(eosinophilic granulocyte, EOS)及嗜酸粒细胞阳离子蛋白(eosinophilic cationic protein, ECP)水平的动态变化规律。方法 前瞻性地测定81例支气管哮喘患儿痰液EOS和ECP,其中24h内有哮喘发作59例(A组),哮喘持续状态22例(B组),另有21例急性上呼吸道感染或急性鼻窦炎患儿作为对照(C组),3组患儿分别于入院即时、72h以及7d采集痰液。结果 A组和B组患儿入院时痰液EOS和ECP的水平明显高于C组(p<0.05),A组和B组患儿入院时痰液EOS水平无统计学差异(p>0.05),而A组患儿入院时痰液ECP水平明显高于B组(p<0.05),A组B组患儿第7d的痰液EOS和ECP水平明显低于入院时(p<0.05)。采用Spearman等级相关分析,患儿痰液EOS与ECP水平存在正相关关系(r=0.726,P<0.05)。结论 联合动态检测痰液EOS和ECP水平可判断小儿支气管哮喘程度,ECP可更敏感预测小儿支气管哮喘转归。 关键词: 支气管哮喘;嗜酸粒细胞阳离子蛋白;嗜酸粒细胞;儿童

The dynamical variation of eosinophilic granulocyte and eosinophilic cationic protein in sputum of children bronchial asthma
Abstract:Abstract Objective To explore varying regularity of eosinophilic granulocyte(EOS) and eosinophilic cationic protein(ECP) in sputum of children bronchial asthma during different physiopathologic states. Methods There were 59 cases(group A) who admitted in because of asthmatic attack in 24 hours, 22 cases(group B) who admitted in because of status asthmaticus, and there were 21 cases(group C) of acute throat infection or acute sinusitis of children, all of whom were detected the level of EOS and ECP when hospitalizing, 72 hours and 7 day after charged in. Results The level of EOS and ECP when hospitalizing was higher in group A and group B than that of group C(p<0.05), there was no difference about EOS between group A and group B(p>0.05), but the level of ECP was higher in group A than that of group B(p<0.05). The level of EOS and ECP were lower 7 days after hospitalizing in group A and group B than that of when charged in(P<0.05). With spearman analysis, the level of EOS in sputum had positive correlation with that of ECP(r=0.726,P<0.05). Conclusions It is valuable to detect EOS and ECP in sputum to judge the severity of children bronchial asthma, and the level of ECP is more sensitive to predict the turnover of children bronchial asthma. 【Key words】bronchial asthma, eosinophilic granulocyte(EOS), eosinophilic cationic protein(ECP), children
