小儿隐匿阴茎的术式选择与疗效分析

目的　探讨小儿隐匿阴茎的术式选择 ,并进行疗效评价。方法　总结近 1 0年来 4 7例隐匿阴茎患儿的临床资料 ,结合临床分型对术式选择及疗效进行回顾性分析。结果　本组 4 7例中 ,轻型 9例 ,随访观察 ;38例中、重型患儿手术治疗 (阴茎体固定术 5例、改良Johnston术 1 2例、Devine术 2 1例 )。术后随访 6月～ 2年。结果表明 :对中型非肥胖患儿改良Johnston术优于阴茎体固定术 (t =1 4 .982 ,P <0 .0 1 ) ,改良Johnston术与Devine术的治疗效果无显著性差异。结论　正确认识隐匿阴茎各型的不同特点是治疗的关键。根据病情和临床分型而选择适当的术式 ,是治疗该类疾病的有效措施。手术最佳时间以 5～ 7岁为宜

The Operation Selection and Therapeutic Analysis of Concealed Penis in Children

Objective To explore the operation alternatives intreating concealed penis in children and to evaluate their respective therapeutic effects. Method 47 cases of concealed penis were treated in the past 10 years .Their clinical data as well as the operation alternatives and their therapeutic effects were retrospectively analyzed. Result 9 cases of slight concealed penis only received follow-up surveys .Of the moderate and severe 38 cases,fixation of the body of penis was used in 5 cases,modified Johnston's operation in 12 cases and Devine's operation in 21 cases respectively.The 6 month to 2 years of follow-up surveys showed that modified Johnston's operation turned out to be more effective than fixation of the body of penis in treating moderate cases (t=14.982,P<0.01), and that there existed no significant differences in therapeutic effects of the modified Johnston' operation and Devine's operation.Conclusion To effectively identify the characteristics of different types of concealed penis is the key to the treatment. Operation alternatives should be properly selected according to patients' conditions and the clinical typing. Operation can be taken when children are 5 to 7 years old.
