逆行置管术治疗泪小管断裂的临床研究

目的对显微镜下泪小管断端吻合加逆行置管术的临床疗效观察。方法在手术显微镜下寻找泪小管断端及吻合,用泪道再通管从鼻腔内进管,经下泪道到上泪道,留管于鼻腔内。结果拔管后随访1年。69例中冲洗泪道通畅者67例,随访期末治愈65例,治愈率94．20％。结论泪道再通管逆行置管术常用于泪道阻塞或泪小管炎者,由于其手术操作简单,术后不影响外观,提高了患者的医从性,临床效果良好。

Observe the effects of antidrome intubation in repair of lacrimal canaliculi laceration

Objective To observe the methods and effects of antidromic intubation in repair of lacrimal canaliculi laceration. Methods To find and anastomosise the canalicular laceration with repass tube under operation microscope, the ends of repass tube were bided in nasal cavity after it was encircled conversly the fulllength of lacrimal pathway. Results The final best correct visual acuity was 0.3 or better in 34 eyes （79.07%）. The lOP were 15.08 ±4.61 mmHg preoperatively and 15.63±3.72 mmHg at the last follow up （ P 〉 0. 5 ）. Postoperative complication include hypertension in 12 eyes, hyphema in 10 eyes, puple deformation in 5 eye, secondary glaucoma in 1 eye, Corneal edema in 10 eyes, Corneal decompensation in 1 eye. There was no retina detachment. Conclusions Antidromic intubation is often used in treating LDOD and lacrimal duct, Especially lacrimal canaliculi obstruction. It has good effects in canalicutar laceration, too. owing to simple operation and without effect for hairdressing, which make it easy to doctor to touch with patients, it is important to resum the function of lacrimal system.
