同型半胱氨酸 超敏C反应蛋白 脉搏波速度与冠心病的关系

摘    要:目的观察同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、臂踝脉搏波传导速度(ba-PWV)与冠心病(CHD)的关系。方法选取2010年3~12月在阳江市人民医院住院的CHD患者87例,其中稳定型心绞痛(SAP)34例,不稳定型心绞痛(UAP)28例,急性心肌梗死(AMI)25例,并设健康对照组(CN)49例,对各组从群进行Hcy、hs-CRP、ba-PWV检测。结果 CHD组Hcy、hs-CRP、ba-PWV较CN组明显升高,两组比较差异有统计学意义(P〈0.05)。CHD各分型组Hcy、hs-CRP、ba-PWV较CN组明显升高,差异有统计学意义(P〈0.05);Hcy在各分型组间比较:AMI组〉UAP组〉SAP组,各组间比较差异具有统计学意义(P〈0.05),hs-CRP与ba-PWV在各分型组间比较AMI组〉UAP组〉SAP组,但AMI组与UAP组比较差异无统计学意义,两组与SAP组比较差异有统计学意义(P〈0.05)。对Hcy与ba-PWV进行相关分析,显示二者呈正相关(r=0.437,P〈0.05)。结论 Hcy、hs-CRP、ba-PWV随着CHD病情严重程度而逐渐升高。因此,以上指标的检测对CHD的诊断及严重程度的判定有一定指导意义。

Abstract:Objective To explore the relationship among high sensitivity C reactive protein(hs-CRP),blood plasma homocysteine(Hcy),Brachial ankle puls wave velocity(ba-PWV) and coronary heart disease(CHD).Methods 87 CHD patients who were treated in the Guangdong Yangjiang people′s hospital from March to December in 2010 were included in this study.87 CHD patients included stable angina pectoris(SAP)34 cases,unstable angina pectoris(UAP)28 cases,acute myocardial infraction(AMI)25 cases.49 cases were selected as the normal control group(NC).All people were measured about hs-CRP,Hcy and ba-PWV.Results To compare with controls,there were higher plasma levels of hs-CRP,Hcy and ba-PWV in patients with CHD(P0.05),the plasma levels of HCY in AMI,UAP and SAP decreases in turn,there were significant differences among three groups(P0.05),there were higher levels of hs-CRP and ba-PWV in AMI and UAP comparing SAP and controls(P0.05).There was no significant difference between UAP and AMI.Hcy had the positive relationship with ba-PWV(r=0.437,P0.05).Conclusion Plasma Hcy,hs-CRP and ba-PWV are correlated to the the severity of CHD,and it is important to the Diagnosis and prognosis of CHD.
