摘    要:目的评价16层螺旋CT(MSCT)后处理技术对胫骨平台骨折(TPF)的诊断分型价值。方法 150例有膝关节外伤史患者先经DR正侧位检查,1周内又经SCT扫描与图像后处理重建。全部患者的TPF按Schatzker分型法分为6型。利用统计学方法比较MSCT与DR对TPF的诊断分型敏感性。结果 DR在150例TPF患者中确诊142例,报告疑似骨折5例,漏诊3例,未能分型。MSCT对150例全部确诊并分型,其中Ⅰ型18例,Ⅱ型48例,Ⅲ型9例,Ⅳ型25例,Ⅴ型36例,Ⅵ型14例。MSCT对TPF诊断分型的敏感性显著高于DR(P〈0.01)。结论 MSCT后处理技术能对TPF作出正确诊断与分型,并能为选择手术方案与路径提供可靠依据。

Abstract:Objective To assess the applied value of 16-slice spiral CT(MSCT) and its postprocessing technigues for diagnosis and typing of tibial plateau fractures(TPFs).Methods 150 patients with traumatic history of knee-joint underwent P-A and lateral digital radiography(DR) following MSCT scaning and image postprocessing reconstruction within one week after DR.TPFs in all patients were classified into six types based on Schatzker typing criterion.The sensitivity of MSCT in the diagnosis and typing of TPFs was statistically analyzed and compared with that of DR.Results Among 150 patients,142 cases with TPF were confirmed by DR,5 suspective and 3 negative,fracture types could not be identified by DR.while MSCT's diagnostic rate for TPF was 100%(150 cases),i.e.18 Type Ⅰ,48 TypeⅡ,9 Type Ⅲ,25 Type Ⅳ,36 Type Ⅴand 14 Type Ⅵ.Conclusion The sensitivity of MSCT in the diagnosis and typing of TPFs was significantly higher than that of DR(P0.01).MSCT and its postprocessing technigues could make definite diagnosis and typing of TPFs and could provide the realible evidence for selecting the suitble surgical protocol.
