高低频硬膜外电刺激治疗创伤后脑水肿的对比研究

摘    要:目的 研究硬膜外高频电刺激和低频电刺激对创伤后脑水肿的作用.方法 108只SPF级SD大鼠采用液压打击制作创伤脑水肿模型并给予硬膜外电刺激,随机分成六组:正常对照组(A组),液压打击组(B组),高频刺激治疗组(C组),低频刺激治疗组(D组),高频刺激对照组(E组),低频刺激对照组(F组).各组依时间点24h,48 h,72 h分别设立亚组.电刺激次数随时间增加而增加.所有大鼠均在末次刺激结束4h断头处死,取打击前区脑组织测脑组织含水量,打击后区作HE 观察.结果 与A组相比,B组脑组织含水量明显增多(P<0.05),C、D、E、F组脑组织含水量未见显著差别(P>0.05);与B组相比,C组、D组脑组织含水量明显减少(P<0.05);D组与C组相比,脑组织含水量未见显著差别(P>0.05),HE染色结果与水肿程度相一致.结论 硬膜外高频和低频电刺激不会引起正常大鼠脑组织脑水肿,两种刺激对创伤后脑水肿均有治疗作用,治疗效果并未随着刺激次数增加而存在差异.

Abstract:Objective To investigate the effect of high frequency and low frequency epidural electrical stimulation (EECS) for brain edema caused by fluid percussion injury (FPI). Methods One hundred and eight Sprague-Dawley rats of specific pathogen free (SPF) standards were randomly divided into six groups: normal (group A), fluid percussion injury (group B), high frequency EECS therapy (group C), low frequency EECS therapy (group D), high frequency EECS control ( group E), and low frequency EECS control (group F). All groups were divided into subgroups according to time points, 24 h, 48 h and 72 h. Times of EECS increased as time prolonged. All samples were cut head to death at 4 h after the last EECS, thereafter the water content of brain tissues in the anterior traumatic zone was examined and the brain tissues of the posterior traumatic zone were used for HE staining. Results Compared with group A, the water content in group B was increased significantly (P 〈0.05) ; while there was no significant difference between group C, group D, group E, group F and group A (P 〉 0.05). Compared with group B, the water content in group C and group D was decreased (P 〈 0. 05 ) and there was no significant difference between group C and group D. Results of HE staining were consistent with the extent of brain edema. Conclusion Both high and low frequency EECS will not cause edema in normal rat brain tissues, and they are effective for the treatment of traumatic brain edema. The efficiency of EECS is not associated with the times increase of EECS.
