小脑实质性肿瘤的MRI诊断与鉴别诊断

目的探讨磁共振成像（MRI）对小脑实质性肿瘤的诊断与鉴别诊断价值。方法回顾性分析经手术病理证实29例小脑实质性肿瘤的MRI表现,所有病例均行横、冠、矢三维的MR平扫及增强扫描。结果 29例肿瘤包括髓母细胞瘤6例,星形细胞瘤5例,血管母细胞瘤4例,转移瘤4例、脑膜瘤3例,室管膜瘤2例,淋巴瘤、黑色素瘤、小脑发育不良性神经节细胞瘤、血管周细胞瘤及孤立性纤维瘤各1例。MR可以清晰地显示小脑实质性肿瘤的部位、大小、形态、信号强度和邻近情况,各种肿瘤均有一定的临床及MRI特征,部分肿瘤表现相似需加以鉴别。结论 MRI对小脑实质性肿瘤的诊断和鉴别诊断有重要的价值,MRI的特征表现结合临床资料可以作出较明确的术前诊断。

Diagnosis and differential diagnosis of MRI for solid tumors in cerebellum

Objective To evaluate the value of MRI for diagnosis and differential diagnosis of solid tumors in cerebellum.Methods 29 patients with solid tumors in cerebellum underwent plain and enhanced MR scanning in axial,coronal and sagittal views.The MRI findings were retrospectively reviewed.All patients were proved by pathology and surgery.Results MRI demonstrate that there were 6 cases of medulloblastomas,5 cases of astrocytomas,4 cases of hemangioblastomas,4 cases of metastatic tumors,3 cases of meningiomas,2 cases of ependymomas,and each 1 cases for lymphoma,melanoma,dysplastic cerebellar gangliocytoma,hemangiopericytoma,solitary fibrous tumors among the 29 solid tumors in cerebellum.The location,size,shape,signal intensity and surrounding condition of solid tumors in cerebellum could be well displayed on MRI.Every kind of tumors had certain clinical and imaging features,some of them were similar and must be differentiated.Conclusion MRI has an important value in diagnosis and differential diagnosis of solid tumors in cerebellum.Typical imaging features of MRI combined with clinical manifestations will be helpful to accurate diagnosis of solid tumors in cerebellum before surgeries.
