心肌缺血总负荷与冠心病相关性研究

目的对"胸痛待查"患者的动态心电图进行分析,探讨心肌缺血总负荷(TIB)的临床意义。方法选择以"胸痛待查"住院的患者,行动态心电图(AECG)及冠状动脉造影(CAG)检查。按CAG结果分为冠心病(CAD)组与非CAD组。对比分析两组间AECG所获得的心肌缺血总负荷指标,研究TIB与冠心病的相关性。结果随冠心病Gensini积分的增加,TIB值增加,提示随着冠状动脉病变严重程度的增加,TIB逐渐增加。TIB筛查CAD的灵敏性为59.6%,特异性为97.9%,符合率为78.8%;Kappa值为0.553。结论TIB与冠心病严重程度呈正相关;TIB筛查冠心病与冠脉造影的符合率较高。

Correlation between total ischemia burden and coronary artery disease

Objective To evaluate the clinical value of total ischemia burden（TIB） by analyzing the results of the ambulatory electrocardiographic（AECG） from the patients with questionable angina symptoms.Methods The patients who had questionable angina symptoms underwent coronary angiography（CAG） and AECG.They were divided into tow groups according to CAG results.To comparatively analyze TIB indicators obtained by AECG in each group,and to evaluate the clinical value.Results With the increase of CAD Gensini scross,TIB value increased.TIB gradually increased with the severity of coronary artery.The sensitivity of screening CAD with TIB was 59.6%,the specificity was 97.9%,and the Kappa value was 0.553.Conclusion TIB is positively correlated with the severity of CAD.The coincidence rate of TIB screening coronary artery disease with CAD is relatively high.
