海水浸泡开放性犬颅脑爆震伤影像学变化及机制研究

摘    要:目的研究海战条件下开放性颅脑爆震伤动物模型颅脑影像学的变化特点。方法以狗颅中线向左(右)1 cm,眶上缘向上1 cm交界处为爆炸中心位置。将15只犬根据爆炸距离(爆炸源最低点距爆炸点的距离)分为3组,分别为3 mm组、3.5 mm组、4 mm组。比较各个组的爆炸效果,选出最适的爆炸距离为3.5 mm。再将20只犬随机分为对照组(非海水浸泡),实验组(海水浸泡)。以爆炸距离3.5 mm致伤后,实验组犬受伤脑组织浸泡于海水环境中,比较两组犬致伤3 h、5 h、8 h后颅脑CT的动态变化。结果经颅脑动态CT观察测量,实验组犬8 h内脑水肿出现时间晚于对照组,同时脑水肿程度明显轻于对照组。结论海水浸泡在8 h内可以明显延缓脑水肿出现。

Abstract:Objective To study the imaging changes of dog models of open craniocerebral explosive injury and an immersion in seawater.Methods The area,1 cm left(right) to the midline of dogs' head and 1 cm up to supraorbital margin,was selected as the explosive site.A total of 15 dogs were divided into 3 groups,3 mm group,3.5 mm group,4 mm group in accordance with the explosive distance(the distance from the lowest point of the explosion ball to the explosive site).The explosive results of three groups were compared,and the appropriate explosive distance was 3.5 mm.A total of 20 dogs were randomly divided into control group(non-seawater immersion) and experiment group(seawater immersion).After explosive injury(3.5 mm to the explosive site),dog heads of the experiment group were soaked in seawater,and the CT images of two groups were compared at 3 h,5 h,8 h post-injury.Results CT images showed the cerebral edema appeared later in experiment group than in control group within 8 h post-injury and the edema degree was the most severe in control group than in experiment group.Conclusion Seawater immersion within 8 h can significantly delay the occurrence of cerebral edema.
