利多卡因联合阿托品与单用地西泮宫颈注射对活跃期停滞的效果比较

目的 比较利多卡因联合阿托品宫颈注射与地西泮宫颈注射对活跃期停滞的干预效果.方法 按照纳入标准,选取2012年3月-10月在四川大学华西第二医院分娩的121例孕妇,对出现活跃期停滞者采用数字随机法分为两组,A组63例给予利多卡因联合阿托品宫颈注射,B组58例给予地西泮宫颈注射,观察两组产程进展及妊娠结局.结果 利多卡因联合阿托品处理宫颈组阴道顺产率68.2％（43/63例）,地西泮处理宫颈组阴道顺产率55.2％（32/58例）,两组差异无统计学意义（P＞0.05）.两组间新生儿窒息率、产后出血率、宫口平均开全时间等比较无显著性差异（P＞0.05）.结论 利多卡因联合阿托品与单用地西泮局部宫颈注射对活跃期停滞均具有较好的效果,两者虽无统计学差异,但利多卡因联合阿托品宫颈注射的效果较地西泮似乎更优.

The effects of lidocaine and atropine cervical treatment or diazepam cervical treatment in labor of active period

Objective To compare the effect of lidocaine and atropine cervical treatment or diazepam treatment during labor of active period.Methods 121 pregnant women were included in the present study.When the active phase was at a standstill,the pregnant women were randomized into two groups：one group was treated cervically with lidocaine and atropine injection （63 cases）,and another with diazepam injection （58 cases）.The labor progress and gestational complications were observed.Results Vaginal birth rate was 68.2％ （43/63） in women treated with lidocaine and atropine,and 55.2％ （32/58） in women treated with diazepam.There was no significant difference between these two groups （P＞ 0.05）.The comparison on neonatal asphyxia rate,postpartum hemorrhage or mean active phase durations also showed no significant difference （P＞0.05）.Conclusion Cervical treatment with lidocaine and atropine injection or diazepam injection both have a good effect on cervical dilations during active period in labor.However,lidocaine and atropine cervical treatment may have a better effect than diazepam treatment.
