肺部弥漫性病变的影像学研究

目的探讨肺部弥漫性病变（Diffuse lung disease，DLD）影像学表现特征。方法回顾分析病理及临床证实的76例有感染性、肿瘤性和心源性肺部弥漫性病变的影像学表现。其中58例行平片和CT检查，12例行CT和HRCT检查，6例行平片、CT和HRCT（high resolution CT）检查。对比分析各种检查技术的诊断价值。结果在76例中，弥漫性肺实质性病变31例（39．6％），弥漫性肺间质性病变25例（32．8％），同时弥漫性累及肺实质与肺间质性病变21例（27．6％）。结论肺部弥漫性病变，HRCT扫描对判定病变范围、部位和性质明显优于胸部平片和常规胸部CT扫描，但没有明确的特征性表现，结合临床和病史对肺部影像学征象进行分析，对提高此类病变的诊断和鉴别诊断具有重要的意义。

Investigation into the imaging fearures of diffuse lung diseases

Objective To investigate the imaging features of diffuse lung diseases for improving the diagnosis of lung diseases. Methods The imaging findings of diffuse lung diseases in 76 cases confirmed by operation and pathology were retrospectively analyzed. The plain film and CT scan were performed in 58 cases. The CT scan and high resolution CT were performed in 12 cases. The plain film and CT scan and high resolution CT were done in 6 cases. The clinical value of various examinaions were assessed. Results The diffuse lung diseases were located in pulmonary diffuse alveolar disease in 31 （39. 6% ） cases, in pulmonary diffuse interstitial disease in 25 （32.8%） cases, all in pulmonary diffuse alveolar disease and pulmonary diffuse interstitial disease in 21 （27.6%） cases. Most diseases were infection, tumors, cor heart. Conclusion High resolution CT is much superior to x - ray plain film. and the CT scan in diagnosis of the range, sites and nature of the disease ~ Imaging incombination with clinial manefastations and history of illness is essential for diagnosis and differential diagnosis of diffuse lung disease.
