动态血压监测在基层医院中的应用价值

摘    要:目的探讨24h动态血压在基层医院中的应用价值。方法对280例动态血压监测者进行临床及结果回顾总结,并按昼夜节律分为4型:杓型、非杓型、反杓型、极度杓型。结果门诊测高血压患者中"白大衣"高血压17例,占10.8%;杓型高血压56例,占35.7%;非杓型血压42例,占26.8%;反杓形高血压35例,占22.3%;极度杓型7例,占4.5%。门诊可疑高血压患者19例,其中11例为隐蔽性高血压。高血压药物治疗检查者104例,血压控制达标62例,达标率59.1%。结论动态血压有利于发现白大衣高血压、隐蔽心高血压,有利于根据昼夜规律用药控制血压,在基层医院中应用动态血压检查手段对于高血压规范管理、提高达标率有着重要作用。

Abstract:Objective Investigate the value of 24 hour ambulatory blood pressure monitoring in the primary hospital.Methods 280 cases with ambulatory blood pressure monitoring are reviewed by clinical and results,according to the circadian rhythm is divided into 4 types: D(Dipper),ND(Non-dipper),AD(Anti-dipper),ED(Extreme dipper).Results Outpatient measured with hypertension: the "white coat" hypertension 17 cases,accounting for 10.8%;D:56 cases,35.7%;ND:42 cases,26.8%;AD:35 cases,22.3%;ED:7 cases,4.5%.19 cases suspected hypertension;hidden hypertension 11 cases;104 cases with hypertension was inspected,62 cases to achieve blood pressure control standards,accounting for 59.1%.Conclusion Ambulatory blood pressure monitoring easy to find the "white coat" hypertension,hidden hypertension,medication to control blood pressure according to the circadian rhythm,ambulatory blood pressure monitoring beneficial to the standardized management of hypertension,it has an important role in the primary hospital management to improve the compliance rate in hypertension standardized management.
