宫颈癌患者血清和组织中胰岛素样生长因子-Ⅱ的表达及临床意义

摘    要:的检测宫颈癌患者血清胰岛素样生长因子-Ⅱ(IGF-Ⅱ)水平及其在肿瘤组织中的表达,探讨IGF-Ⅱ在宫颈癌发生、发展中的作用。方法采用放射免疫法检测血清IGF-Ⅱ水平,应用免疫组织化学方法检测IGF-Ⅱ在正常宫颈组织和宫颈癌组织中的表达情况,同时对组织中IGF-Ⅱ表达与宫颈癌各临床病理参数之间的关系进行分析。结果(1)宫颈癌组和对照组血清IGF-Ⅱ浓度分别为(1.98±0.53)和(0.51±0.16)μg/L,二者相比差异有统计学意义(P〈0.01);宫颈癌患者治疗后血清IGF-Ⅱ浓度比治疗前明显降低,淋巴结转移组血清IGF-Ⅱ浓度为(2.26±0.32)μg/L,明显高于淋巴结未转移组(1.734-0.42)μg/L,差异有统计学意义(P〈0.01)。(2)宫颈癌、正常宫颈组织中IGF-Ⅱ表达阳性率分别为73.3%和10.0%,二者相比差异有统计学意义(P〈0.01)。IGF-Ⅱ的阳性表达与宫颈癌临床分期、分化程度呈显著相关,差异有统计学意义(P〈0.01),其在淋巴结转移组的阳性表达率明显高于无淋巴结转移组(x^2=8.69,P〈0.01)。结论IGF-Ⅱ的高表达与宫颈癌的发生、发展及侵袭转移有关,检测IGF-Ⅱ能为宫颈癌的临床诊断和预后观察提供有意义的参考。

Abstract:Objective To investigate the expression of serum and tissue insulin-like growth factor Ⅱ (IGF-Ⅱ ) in patients with cervical cancer, and to evaluate its clincial significance in the ocuurence and development of cervical cancer. Methods The serum level of IGF- Ⅱ was examined by radioimmunoassay (RIA), and the expression level of IGF- Ⅱ in the tumor and normal tissue was detected with immunohistoehemical technique. The relationship of tissue expression level of IGF-Ⅱ and the clinicopathological parameters was analyzed as well. Results The serum IGF-Ⅱ levels in cervical cancer group and the control group were (1.98±0.53)μg/L and (0.5±0.16)μg/L, respectively. There was significant difference between them (P〈0.01) ; the serum IGF- Ⅱ level was decreased significantly after therapy. The serum IGF- Ⅱ level in lymph node metastasis group was obviously higher than that in lymph node nonmetastasis group (2.26±0.32)μg/L vs (1.73±0.42) μg/L,P〈0.01]. The positive expression rate of IGF-Ⅱ in cervical cancer tissues was 73.3%, significantly higher than that in normal cervical tissues (10.0%) (P〈0.01). The positive expression of IGF- Ⅱ was closely correlated with different clinical stage and differentiation degree of carcinoma (P〈0.01). The positive expression rate of IGF-Ⅱ in lymph node metastasis group was significantly higher that in lymph node non-metastasis group (2 = 8.69, P〈0.01). Conclusion The excessive expression of IGF-Ⅱ may be correlated with the genesis, invasion and metastasis of cervical cancer. The expression of IGF- Ⅱ may be useful marker of diagnosis and prognosis evaluation of cervical cancer.
