经腋下小切口治疗自发性气胸180例

经腋下小切口治疗自发性气胸１８０ 例。经胸腔穿刺胸腔闭式引流无效而行手术治疗１２６ 例。首次发病直接手术治疗３８ 例。双侧性病变８ 例，其中３ 例同时行双侧手术。肺大泡缝扎或结扎术１３２ 例，胃肠吻合器缝合６ 例，剖胸止血３０ 例。术中平均输血３００ｍｌ。手术时间平均５６ｍｉｎ，术后胸腔引流液平均１０５ｍｌ。术后置管１～１１ｄ，平均２．１ｄ 拔管，住院日８ ．６ｄ。术后出血１ 例，复发１ 例均经再次手术治疗。术后１５～３０ｄ 恢复上学或工作，认为腋下小切口手术适用于所有自发性气胸患者。

Spontaneous Pneumothorax Were Treated with Transaxillary Minithoracotomy

Amony one hundred and eighty cases of spontaneous pneumothorax 30 were hemopneumothorax, 35 recurrent pneumothorax. One hundred and twenty six patients were treated by opreation because of the failure of thorax (carity) puncture and thoracostomy tube drainage. Thirty eight patients were directly treated by operation, 8 were bilateral pneumothorax, 3 of 8 patients were bilaterally operated at the same time, 132 patients were treated with pulmonary bulla sutured or tied, of them 6 by GIA stapler, 30 were conducted hemostasis with thoracotomy. Mean volume of blood tramsfusion during the operation was 300ml. Mean duration of the operation was 56 minutes. Mean volum 1 of post-operation drainage was 105ml. Tube was removed 1 to 11 days after operation, mean 2.1 days. Mean hospital stay was 8.6 days. One patient bleeded after operation. One recurrent patient took second operation.
