超声心动图评价心力衰竭患者心脏再同步化治疗的短期疗效

目的探讨超声心动图在充血性心力衰竭患者心脏再同步化治疗（CRT）短期疗效评价中的应用价值。方法选取19例接受CRT的心力衰竭患者，采用超声心动图检测量术前、术后1个月患者的左心室舒张末、收缩末直径（LVDD、LVDS）和容积（LVEDV、LVESV），双平面Simpson法测量左心室射血分数（LVEF），彩色多普勒测量二尖瓣反流面积（MRA），并在连续多普勒频谱图上测量二尖瓣反流上升支1m／s和3m／s两点之间的时间（△t），由此估测左心室dp／dt。结果心力衰竭患者CRT术后，发生左心室逆重构，LVDD、LVDS及LVEDV、LVESV明显小于术前（P〈0．01或P〈0．05）；左心室收缩力增强，LVEF明显提高（P〈0．01），MRA减少（P〈0．05），左心室dp／dt明显提高（P〈0．01）。结论超声心动图常规参数及do／dt是评价心力衰竭患者CRT术后短期疗效的简便、有效指标。
