急性心肌梗死并重度泵衰竭早期应用主动脉内球囊反搏对C-反应蛋白的影响

摘    要:目的评价急性心肌梗死(AMI)并重度泵衰竭的患者早期应用主动脉球囊反搏术(IABP)对C反应蛋白(CRP)的影响。方法选取AMI并重度泵衰竭、入院后6 h内无心源性休克发生的患者共54例,其中IABP组27例,在早期直接PCI治疗同时应用IABP辅助治疗;对照组27例,给予直接PCI及药物治疗。比较两组患者入院时、发病14 d及随访6月时CRP、高敏CRP(hs-CRP)及预后。结果 IABP组与对照组入院时CRP和hs-CRP水平无显著差异(P>0.05),但发病14天及6个月后IABP组CRP和hs-CRP水平显著低于对照组(P<0.05),IABP组预后显著优于对照组。结论早期应用IABP能显著改善AMI并重度泵衰竭患者预后,至少部分与其降低全身炎症反应有关。

Effect of early usage of intra-aortic balloonpump on C reaction protein in patients with acute myocardial infarction complicated by pump failure

Abstract: Objective ] To investigate the effect of early usage of intra-aortic balloonpump (IABP) on G reaction protein in patients with acute myocardial infarction (AMI) complicated by grave pump failure. Methods ] 54 patients with AMI complicated by grave pump failure underwent primary PCI were randomly divided into two groups: the IABP group (N=27) received PCI treatment with early IABP support, and the control group (N=27) received PCI treatment without IABP support. The serum CRP and hs-CRP levels were measured on admission, at 14th day and 6th month. Results ] The serum CRP and hs-CRP levels did not show significant differences between the two groups on admission (P〉0.05). However, at 14th day and sixth month, compared with control group, the IABP group showed significantly decreased CRP and hs-CRP levels (P〈0.05) and better prognosis. Conclusion ] Early usage of intra-aortic balloonpump (IABP) improved prognosis in patients with acute myocardial infarction (AMI) complicated by grave pump failure, at least partly associated with decreased systemic inflammatory response.
