Y形耳甲软骨移植矫正唇裂继发鼻畸形

目的 寻求一种矫正唇裂术后继发鼻畸形的理想手术方法。 方法 切取2.5 cm×1.2 cm大小的耳甲软骨制备成Y形支架移植于鼻小柱,加强鼻尖软骨支撑,同时广泛游离双侧鼻翼软骨后与Y型软骨支架上部缝合固定。 结果 11例单侧唇裂术后继发鼻畸形均获得了满意的效果。鼻尖低平得以抬高;患侧鼻翼圆钝矫正,与健侧高度基本相同;鼻小柱旁曲侧偏纠正,鼻端外形明显改善。随访6~24个月,鼻端形态稳定,无再发畸形,耳廓供区无变形。 结论 Y形耳甲软骨移植矫正唇裂鼻畸形是一种理想的治疗方法。

Application of Y Shape Ear Cartilage Transplantation in Correction of the Unilateral Cleft Lip Nasal Deformity

Objective To search an ideal operation method of secondary unilateral cleft lip nasal deformity correction. Methods After cutting down a rectangular cartilage which was in the size of 2.5cm×1.2cm, split the upper one third alone the lone axis and folded it. Then it was straightened in the shape of "Y". The stalk portion was sutured between the medial crura, and the upper portion was sutured to the dome of the alar cartilages. Results 11 cases were included. Among them, 5 patients were very satisfied, 4 patients were satisfied, and 2 patients felt they were improved. The nasal apex and alae nasi on the affected side was raised. The collapse of columella nasi got improved. After follow-up between 6-24 months, the shape of nasal was stable. No case recurred, and no deformity happened on donor sites. Conclusion Unilateral cleft nasal deformity correction using a Y shaped cartilage is an ideal treatment method.
