实时三维子宫输卵管超声造影与X碘油造影在输卵管源性不孕症中的一致性研究

摘    要:目的探讨经阴道实时三维子宫输卵管超声造影(RT-3D-HyCoSy)在输卵管源性不孕症中的临床应用价值及与X碘油造影评估输卵管通畅性的一致性.方法选取2017年8月至2018年7月在桂林医学院附属医院考虑为输卵管源性不孕症患者18例为研究对象,患者分别进行实时三维子宫输卵管超声造影(RT-3D-HyCoSy)及X线下碘油造影(HSG),对比观察两组输卵管走形及盆腔造影剂分布情况,比较RT-3D-HyCoSy、HSG对输卵管通畅性的诊断价值的一致性.结果 18例共36条输卵管,RT-3D-HyCoSy显示输卵管通畅21条(58.3%),输卵管阻塞10条(27.8%),输卵管通而不畅5条(13.9%),HSG显示输卵管通畅21条(58.3%),输卵管阻塞9条(25.0%),输卵管通而不畅6条(16.7%),显示两种检查方法检查结果具有较好的一致性(Kappa=0.76),且RT-3D-HyCoSy可清晰显示宫腔病变3例,其中,子宫内膜息肉2例,宫腔粘连1例.结论 RT-3D-HyCoSy具有可动态立体显示子宫输卵管情况、无辐射、无碘过敏等优点,在诊断输卵管源性不孕症中操作便捷、安全、准确,值得临床推广.
