二尖瓣成形术——附14例报告

背景 ]探讨二尖瓣成形术术式及手术时机的选择问题 .病例报告 ]对从 1999年 2月至 2 0 0 2年 5月期间施行的 14例二尖瓣成形术进行临床分析 ,其中二尖瓣交界部切开 4例 ,乳头肌切开及腱索切除 2例 ,二尖瓣前叶切除并交界部折叠术 1例 ,二尖瓣后叶切除并瓣环成形术 3例 ,腱索缩短术 2例 ,人工腱索形成术 1例 ,切除腱索及钙化灶 1例 .全组无 1例手术死亡病例 .1例成形术后改做二尖瓣置换术 ,2例瓣膜病变较重 ,术后仍有Ⅰ～Ⅱ级收缩期杂音 .讨论 ]手术方法不同 ,术后效果有所差异

Mitral valvuloplasty--14 cases of report

BACKGROUNDTo study the mitral valvuloplasty and the selection of operation opportunity.CASE REPORTThe 14 cases of mitral valvuloplasty were retrospectively analyzed from February, 1999 to May, 2002, in which the patients with different lesions of mitral valve were performed different operations: boundary part of mitral valve was excided in 4 cases, the resection of papillary muscles with chordae tendinae was performed in 2 cases, resection of anterior leaflet with commissural plication in 1 case, resection of posterior leaflet with annuloplasty in 3 cases, the shortening of chordae tendon in 2 cases, artificial chordae tenoplasty in 1 case and resection of calcified chordae tendinae in 1 case. There has no one case dead at operation, but one case was changed the valvuloplasty to mitral valve replacement and 2 cases with serous mitral valvular lesion still had I～II degree systolic murmur after operation.DISCUSSIONPostoperative effects are different following with different operative methods for the mitral valvuloplasty.
