彩色多普勒超声在颈部淋巴结病变中的诊断价值

目的 探讨彩色多普勒超声在颈部淋巴结病变中的诊断价值.方法 回顾性分析97例经手术病理证实的颈部淋巴结的彩色多普勒超声图像特征,包括收缩期最大血流速度（PSV）、舒张末期血流速度（EDV）、血流阻力指数（RI）及血流频谱形态.根据病理结果将淋巴结分为良性组（60个）和恶性组（237个）.结果 ①良恶性组的PSV和EDV比较差异无统计学意义（P＞0.05）.②良恶性淋巴结RI取最低值时,差异无统计学意义（P＞0.05）;取最平均值时,恶性组大于良性组,差异有统计学意义（P＜0.05）;取最大值时,恶性组明显大于良性组,P＜0.001,说明RI在取最大值时对良恶性淋巴结的鉴别有重要的意义.③两组间频谱形态比较,差异有统计学意义.结论 彩色多普勒超声可为颈部淋巴结病变的鉴别诊断提供可靠的信息

Value of color Doppler ultrasound in diagnosis of cervical lymph nodes diseases

Objective To explore the value of color Doppler ultrasound in diagnosis of cervical lymph nodes diseases. Methods Ninety - seven cases of cervical lymph nodes diseases were confirmed by surgery and pathology, their sonogram features were analyzed retrospectively, including peak systolic velocity（PSV） , end - diastolic velocity（ EDV）, RI and blood flow spectrum. According to the pathological results, the patients were divided into benign group （60 cases ） and malignant group （ 237 cases）. Results There was no significant difference of PSV and EDV in the two groups （ P 〉 0.05 ）. When the lowest value of RI was taken, there was no significant difference between the two groups （ P 〉 0.05 ） , when mean value of RI was taken, there was significant difference between the two groups （ P 〈 0.05 ）, when the max of RI was taken, it has important significance in differential diagnosis of benign and malignant lymph nodes （P 〈 0.001 ）. While there was significant difference of blood flow spectrum in the two groups. Conclusion Color Doppler ultrasound can provide reliable information for diagnosis of cervical lymph nodes diseases.
