超声心动图诊断婴幼儿心内膜弹力纤维增生症

摘    要:目的 探讨超声心动图在婴幼儿心内膜弹力纤维增生症(EFE)诊断与鉴别诊断中的价值.方法 对30例 EFE患儿(EFE组)进行超声心动图检查,超声测量房室内径、心内膜厚度、瓣膜血流及心功能指数.另选同期健康婴幼儿30例为对照组.结果 超声显示EFE组有心内膜增厚、回声增强,二尖瓣瓣膜反流,左室收缩功能明显减低;左室舒张末内径(LVDd)扩大,而射血分数(EF)、心室短轴缩短率(FS)、每搏指数(SI)、心脏指数(CI)均降低,与对照组比较差异均有统计学意义(P<0.05);峰值速度比(E/A)、等容舒张时间(IVRT)两组间比较差异均无统计学意义(P>0.05).结论 超声心动图对EFE的早期诊断及疗效评价具有重要的临床价值.

Abstract:Objective To investigate the value of echocardiography in diagnosis and differential diagnosis of endocardial fibroelastosis (EFE) in children. Methods Thirty children with EFE were examined by echocardiography. The heart chamber size, endocardium thickness, blood flow and indexes of ventricular function were measured. Then 30 normal children were as control group ( NC ). Results Uhrasonography showed thickening of the endocardium with enhanced echo, mitral valve regurgitation, and obviously decreased left ventricular systolic function. While the left ventricular end -diastolic diameter(LVDd) was increased, ejection fraction(EF) , fractional shortening(FS) , pulsate index(SI) and cardiac index(CI) were decreased. There was significant difference between EFE group and control group ( P 〈 0.05 ) , while there was no significant difference of E/A ratio and isovolumic relaxation in the two groups ( P 〉 0. 05 ). Conclusion Echocardiography has important clinical value in early diagnosis and therapeutic effect evaluation of EFE.
