彩色多普勒超声对胡桃夹综合征的诊断价值

目的 探讨彩色多普勒超声对胡桃夹综合征的诊断价值.方法 对15例胡桃夹综合征患者,平卧位时测量左肾静脉受压狭窄部位内径（b）及受压扩张部位内径（a）,脉冲多普勒测量受压部位血流速度（Vb）及扩张部位血流速度（Va）;站立背伸位15 min后重复测量上述指标.计算两种体位下a/b、Vb/Va及最窄处压力阶差（△P）.结果 不同体位下,15例胡桃夹综合征患者的 LVR的a均较b明显增宽,Vb均较Va明显增快,△P均升高;平卧位时a/b＞3,Vb/Va＞4,△P＞3.5,站立背伸位15 min后,a/b＞4,Vb/Va＞6,△P＞6,站立背伸位15 min后与平卧位各参数的比较差异均有统计学意义（P＜0.05）.结论 彩色多普勒超声检查可作为胡桃夹综合征的首选影像学检查方法.

Value of color Doppler ultrasound in diagnosis of nutcracker syndrome

Objective To explore the value of color Doppler ultrasound in diagnosis of nutcracker syndrome. Methods Fifteen Patients with nutcracker syndrome were enrolled in this study. The inner - diameter of the narrow （b） and dilated （a）portion of the compressed left renal vein was measured, the blood flow velocity of narrow （Vb） and dilated （Va） portion was measured by pulse Doppler, 15 min after extending back on standing position, the parameters were measured again. The a/b,Vb/Va and pressure gradient AP of the most narrow part were calculated. Results In differenct positions, b was obviously wider than a , Vb was increased faster than Va, △P was increased. In dorsal decubitus position, a/b 〉3, Vb/Va 〉 4 and △P 〉3.5 were shown; 15 min after extending back on standing position, a/b 〉4,Vb/Va 〉6 and AP 〉6 were shown, there was significant difference of the parameters between different positions （ P 〈 0.05 ）. Conclusion Color Doppler ultrasound has, high accuracy and important clinical value, it can be the first method for diagnosis of nutcracker syndrome.
