摘    要:目的探讨肾综合征出血热(HFRS)血清Th1/Th2代表性细胞因子IL-12在发病中的作用。方法对25例HFRS患者(轻症组11例;重症组14例)按病期采集血液,用ELISA法检测白细胞介素-12p70(IL-12p70)并同步检测血象和BUN。结果从发热期至多尿期血清IL-12p70含量均见升高,轻症组之峰值为(1.42±1.10)μg/L,重症组之峰值为(2.11±2.13)μg/L,皆显著高于对照组(0.56±0.10)μg/L水平,P<0.01。IL-12之变化曲线与血尿素氮(BUN)的变化趋势相一致,而与血小板(BPC)的变化趋势相反。结论 HFRS病程中IL-12大量释放,导致Th1/Th2细胞平衡偏移,产生多器官免疫病理损伤。对症积极治疗可改善本病预后。

Abstract:Objective To investigate the changes and pathogenic significance of IL-12(maybe resposible for serum Th1/Th2 cells) in the genesis of hemorrhagic fever with renal syndrome(HFRS).Methods 25 patients with HFRS were divided into either moderate group(11 cases) or severe group(14 cases) according to the severity of illness.Serum levels of interleukin-12p70(IL-12p70) were detected by enzyme-linked immunosorbent assay(ELISA),blood urea nitrogen(BUN) and platelet by automatic biochemical analyzer and blood analyzer.Results The serum levels of IL-12 p70 with a peak value [(1.42±1.10)μg/L] in moderate group and with a peak value [(2.11±2.13)μg/L] in severe group were significantly increased during the first stage of HFRS(from fever to polyuria stages) compared with control group [(0.56±0.10)μg/L],P<0.01.The curve of IL-12 was similar to that of BUN but was contrary to blood platelet count.Conclusion The massive release of interleukin-12 in the course of HFRS may cause the Th1/Th2 cells balanced displacement and result in the multi-organ immunity pathological damage.The active expectant treatment may improve this disease prognosis.
