社区流动人口围产儿死亡原因分析

目的分析社区流动人口围产儿死亡原因并探讨其死亡相关因素,以提高围产期保健工作质量,降低围产儿死亡率。方法对佛山市禅城区澜石医院68份于2000～2009年期间死亡的围产儿住院病历进行统计分析。结果 10年间围产儿死亡68例,均集中于流动人口,死亡率为7.78%。围产儿死因分类中,死胎占61.76%,死产占14.71%,早期新生儿死亡占23.53%,死胎明显高于死产和早期新生儿死亡(χ2=39.3846,P<0.01;χ2=23.3103,P<0.01);死亡原因以先天畸形、脐带因素、母体因素为主要原因;接受产检5次以上的围产儿死亡率明显低于5次以下,有显著性差异(χ2=67.7647,P<0.01)。结论重视社区流动人口孕产妇保健管理,扩大免费产检、婚检目标人群,增加社区医院先进设备的投入,提高社区医疗单位产科质量,来预防早产及其并发症的发生,是降低流动人口围产儿死亡率的关键。

The Analysis of Death Reason for Perinatal Children of the Community Floating Population

Objective To analyze the reason of death perinatal children and discuss its death related factor of the community floating population to improve the perinatal health care work quality and reduce perinatal mortality rate.Method Statistical analysis was carried on with 68 medical record cases of death perinatal children during 2000 ~ 2009 at Lanshi hospital of Chancheng district,Foshan.Result During 10 years,there were 68 cases died and concentrated in the floating population,the mortality rate was 7.78 %.Classifying the reason of perinatal mortality children,the stillborn foetus took up 61.76 %,the stillbirth was 14.71 %,the death early newborn took up 23.53 %,the stillborn foetus rate was obviously higher than the others(χ2 = 39.3846,P <0.01;χ2 = 23.3103,P <0.01).The primary reason of death were congenital malformation,umbilical cord factor and parent factor.The perinatal mortality rate which had prenatal examination above 5 times lower than those below 5 times sobviously,it had a significant difference(χ2 = 67.7647,P <0.01).Conclusion It is the key point to reduce the rate of floating population's perinatal mortality by attaching the importance to the health care management of the community floating population's pregnant and lying-in women,enlarging the target population of free prenatal examination and premarital checkups,increasing the investment in advanced equipment of the community hospital,improving the medical service quality of the community's obstetrical department and preventing premature birth and the occurrence of complications.
