骨髓切片与涂片联合应用于骨髓造血异常综合征诊断探讨

收集了从１９９０年６月～１９９３年５月经临床、外周血象、骨髓涂片和骨髓活检确诊为骨髓造血异常综合征（ＭＤＳ）的３１例患者资料，进行了研究观察。外周血和骨髓涂片根据法美英协作组（ＦＡＢ）分型标准，骨髓活检采用塑料包埋半薄切片。我们认为组织病理形态分类对以涂片为基础的ＦＡＢ分型是很好的补充，两者相结合诊断ＭＤＳ是一作常有效的方法。

Combined use of bone marrow smears and sections: morphologic classification of myelocloclyeplastic synolromes

he clinical features,bone marrow aspirates and biopsies were studied in 31 patients with MDS from June of 1990 to May of 1993.Smears of peripheral blood films and bone marrow a spirates were stained according to Wright.The sections of undecalcifled plastic embedded biopsies were stained ac-cording to HE HGF Gomori and Masson.It was found more reliable and accurate for the diagnosis of MDS to use the combination of aspirate and biopsy analyses than to use either ofthem alone especially for the difficult-to-diagnose cases due to cytopenia and unsuccessful bone marrow aspirates(dry tap or dilution).A bone marrow biopsy was necessary The hypercellular,disorganization of bone marrow topography,and stromal changes were the main morphologic characteristics According to bone mar-row morphology,7 subtypes were divided:(1) Megaloblastoid(2) Sideroblastic(3) Proliferative(4)Blastic,(5)Hypoplastic.(6) Fibrotic(7)Inflammatory The results suggested that the histomorphologic classification was a complement to that of the FAB group based on the smears and the combined utili-zation could serve as an assistance to improve the rates ofcorrect diagnosis and prognosis
