地塞米松局部注射联合L-甲状腺素治疗甲状腺机能减退的疗效观察

目的 :观察地塞米松局部注射联合L -甲状腺素(LT4)与口服L -甲状腺素治疗桥本病所致青少年甲减的近期疗效。方法 :38例桥本病所致青少年甲减患者随机分为两组 ,地塞米松局部注射联合L -甲状腺素组20例 ,口服L -甲状腺素组18例 ,随访治疗2年。结果 :地塞米松局部注射联合L -甲状腺素组从治疗到血清TSH降至正常所需时间短 (P<0.05) ,LT4 剂量小 (P<0.01) ,抗体(TgAb、TmAb、TpoAb)转阴率高 (P<0.05) ,甲状腺肿大消失率高 (P<0.05)。结论 :地塞米松局部注射联合L -甲状腺素治疗桥本病所致青少年甲减的近期疗效好 ,不良反应少。

Observation of curative effect on local injection of dexamethasone combined with levo- thyroxine for treating hypothyroidism

Objective:To observe the recent curative effects of local injection of dexamethasone combined with levo-thyroxine and oral levo-thyroxine for treating adolescent hypothyroidism caused by Hashimoto's disease.Methods:38 patients with adolescent hypothyroidism caused by Hashimoto's disease were randomized into two groups,and treated for two years with either local injection of dexamethasone combined with levo-thyroxine(n=20) or oral levo-thyroxine(n=18) respectively.Results:In the combined group.the time for serum TSH restored to normal was short,the dosage of LT4 was small,the rate of antibody(TgAb,TmAb,TpoAb)converted to negative was high,the rate of thyroid enlargement recovery was high comparing with those in the oral group respectively(P<0.05,P<0.01,P<0.05,P<0.05).Conclusion:The recent curative effect of local injection of dexamethasone combined with levo-thyroxine for treating adolescent hypothroidism caused by Hashimoto's disease is favourable with less side-effect.
