针药并用床边徒手置入鼻肠管在人工气道患者中的应用

目的观察针灸配合促胃动力药(甲氧氯普胺注射液)对人工气道危重症患者床边徒手鼻肠管置入的影响。方法选取人工气道106例需行肠内营养支持的危重症患者,床边徒手置入螺旋型鼻肠管,跟踪观察置管后48 h和72 h后鼻肠管尖端在位情况。采用随机对照方法将患者分为对照组(单纯促胃动力药)、观察组(针灸配合使用促胃动力药)。统计分析两组的置管成功率。结果对照组的置管成功率为24.0%(12/50),观察组为98.2%(55/56),两组置管成功率比较差异有统计学意义(P<0.05)。结论针药联合可显著提高螺旋型鼻肠管的置管成功率,为人工气道患者肠内营养的早期开通提供切实依据。

Application of Combined Acupuncture and Medicine to Bedside Manual Placement of a Nasointestinal Tube in Patients with Artificial Airway

Objective To investigate the influence of acupuncture and moxibustion plus gastric motility drug(Metoclopramide Injection)on bedside manual placement of a nasointestinal tube in seriously-ill patients with artificial airway.Method One hundred and six seriously-ill patients with artificial airway requiring enteral nutrition support were enrolled and given bedside manual placement of a spiral nasointestinal tube.The position of the tip of the nasointestinal tube was observed by following up at 48 and 72 hrs after the tube placement.The patients were assigned,using a randomized controlled approach,to control(gastric motility drug alone)and observation(acupuncture and moxibustion plus gastric motility drug)groups.The success rate of the tube placement was statistically analyzed in the two groups.Result The success rate of the tube placement was 24.0%(12/50)in the control group and 98.2%(55/56)in the observation group.There was a statistically significant difference in the success rate of the tube placement between the two groups(P<0.05).Conclusion Combined acupuncture and medicine can markedly increase the success rate of spiral nasointestinal tube placement.It provides a practical basis for early enteral nutrition in patients with artificial airway.
