针刺联合多潘立酮治疗脓毒症急性胃肠损伤疗效观察

目的观察针刺联合多潘立酮治疗脓毒症急性胃肠损伤的临床疗效。方法 98例脓毒症急性胃肠损伤患者随机分为研究组和对照组,每组49例。对照组采用多潘立酮治疗,研究组采用针刺联合多潘立酮治疗。观察并比较两组治疗前后腹内压、肠鸣音变化,比较两组的生存率、不良反应发生率及临床疗效。结果研究组总有效率(93.9%)显著高于对照组(75.5%)(χ~2=6.376,p=0.012)。两组治疗第3天、治疗第7天腹内压均较同组治疗前明显降低,肠鸣音较同组治疗前明显升高(P<0.05),且研究组腹内压、肠鸣音均优于对照组(P<0.05)。两组生存率及不良反应发生率比较,差异均无统计学意义(χ~2=0.710,P=0.399;χ~2=0.211,P=0.646)。结论针刺联合多潘立酮能明显改善脓毒症急性胃肠损伤患者肠鸣音次数以及异常的胃肠道动力,并使异常的肠鸣音逐渐恢复正常,且安全性高。

Therapeutic Observation of Acupuncture plus Domperidone for Acute Gastrointestinal Injury in Sepsis

Objective To observe the clinical efficacy of acupuncture plus Domperidone in treating acute gastrointestinal injury in sepsis.Methods A total of 96 sepsis patients with acute gastrointestinal injury were randomized into a research group and a control group,with 49 cases in each group.The control group was treated with Domperidone,and the research group was intervened by acupuncture plus Domperidone.The intra-abdominal pressure and bowel sound were observed and compared before and after treatment.The survival rate,adverse reaction rate and clinical efficacy were compared.Result The total effective rate of the research group(93.9%) was markedly higher than that of the control group(75.5%)(χ~2=6.376,P=0.012).Compared with the baseline,the intra-abdominal pressure declined and bowel sound increased significantly on the 3 rd and 7 th treatment days in both groups(P<0.05),and the intra-gastric pressure and bowel sound in the research group were better than those in the control group(P<0.05).There were no significant differences in the survival rate and adverse reaction rate between the two groups(χ~2=0.710,P=0.399;χ~2=0.211,P=0.646).Conclusion Acupuncture plus Domperidone can significantly improve the bowel sound count and gastrointestinal motility,and gradually restore normal bowel sound in sepsis patients with acute gastrointestinal injury,and it is a highly safe approach.
