艾灸预防性治疗季节性变应性鼻炎临床观察

目的观察艾灸预防性治疗季节性变应性鼻炎(SAR)的临床疗效。方法将85例符合标准的SAR患者按照随机数字表法分为艾灸组(28例)、西药组(29例)和对照组(28例)。艾灸组采用温和灸与灸盒灸相配合方式治疗;西药组采用布地奈德鼻喷雾剂治疗;对照组不予任何处理,定期随访。艾灸组及西药组患者治疗时间均选择从4月花粉伊始期前2周进行,4周后结束。采用视觉模拟量表(VAS)及鼻结膜炎生活质量量表(RQLQ)于花粉伊始期(治疗2周后),花粉高峰期(治疗4周后)及花粉消散期(治疗后4周)比较各组的评分情况。结果与对照组比较,艾灸组及西药组治疗后流涕、鼻塞、喷嚏、鼻痒、眼痒症状改善(P<0.01,P<0.05);艾灸组在花粉伊始期流涕、鼻塞、喷嚏、鼻痒、眼痒症状评分与西药组比较,差异无统计学意义(P>0.05);艾灸组在花粉高峰期流涕、喷嚏及鼻痒评分和花粉消散期流涕、鼻塞、喷嚏、鼻痒、眼痒评分优于西药组(P<0.05)。与对照组比较,艾灸组及西药组治疗后生活质量评分改善(P<0.01,P<0.05);艾灸组在花粉伊始期和花粉消散期生活质量评分与西药组比较,差异无统计学意义(P>0.05);艾灸组在花粉高峰期睡眠、非鼻/眼症状及行为问题评分优于西药组(P<0.05)。结论季节性变应性鼻炎患者在花粉期来临前采用预防性治疗能显著改善花粉致敏症状及生活质量。艾灸疗法与鼻内皮质类固醇治疗近期效果作用相当,但远期效果优于鼻内皮质类固醇。

Clinical Observation of Moxibustion in Preventive Treatment of Seasonal Allergic Rhinitis

Objective To observe the clinical efficacy of moxibustion in preventive treatment of seasonal allergic rhinitis(SAR).Method Eighty-five eligible SAR patients were divided into a moxibustion group(28 cases),a Western medication group(29 cases)and a control group(28 cases)using the random number table method.The moxibustion group was intervened by mild moxibustion in combination with moxa-box moxibustion;the Western medication group was treated with Budesonide nasal spray;the control group did not receive any intervention.Follow-ups were conducted regularly.Treatment for patients in the moxibustion group and the Western medication group started 2 weeks prior to the beginning of the pollen season in April,lasting for 4 weeks.The three groups were compared using Visual Analogue Scale(VAS)and Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)at the beginning of pollen season(after 2-week treatment),the peak of pollen season(after 4-week treatment)and the end of pollen season(4 weeks after treatment).Result Compared with the control group,the moxibustion group and the Western medication group showed improvements in symptoms including runny nose,stuffy nose,sneezing,itching nose and itching eyes after treatment(P<0.01,P<0.05);the symptom scores of runny nose,stuffy nose,sneezing,itching nose and itching eyes in the moxibustion group were insignificantly different from those in the Western medication group at the beginning of pollen season(P>0.05);the moxibustion group was superior to the Western medication group comparing the symptom scores of runny nose,sneezing and itching nose at the peak of pollen season and the scores of runny nose,stuffy nose,sneezing,itching nose and itching eyes at the end of pollen season(P<0.05).Compared with the control group,the moxibustion group and the Western medication group showed improvement in the quality of life score after treatment(P<0.01,P<0.05);there was no significant difference in the quality of life score between the moxibustion group and the Western medication group at the beginning and the end of pollen season(P>0.05);the moxibustion group was superior to the Western medication group comparing the scores of sleep,non-nasal/eye symptoms and behaviors at the peak of pollen season(P<0.05).Conclusion Preventive treatment implemented prior to the beginning of pollen season can significantly improve the pollen-triggered symptoms and quality of life of SAR patients.Moxibustion can produce equivalent short-term efficacy and more significant long-term efficacy compared with intranasal corticosteroid treatment.
