三维适形放疗联合介入和单纯介入治疗原发性肝癌疗效比较

摘    要:目的比较三维适形放疗联合介入和单纯介入治疗原发性肝癌的疗效。方法 54例原发性肝癌患者随机分为三维适形放疗+介入组和单纯介入组,所有患者均行两次介入,三维适形放疗+介入组加三维适形放疗,按照WHO肿瘤评定标准评价近期疗效,以3年内生存率评价远期疗效。结果 54例原发性肝癌患者全部完成治疗,3-DCRT+TACE组中27例患者CR+PR19例,有效率70.4%;TACE组中,CR+PR15例,有效率55.56%。3-DCRT+TACE组中1、2、3年生存率分别是74.1%、44.4%及29.6%;TACE组中1、2、3年生存率分别是59.3%、33.3%及18.5%。结论 TACE联合3-DCRT治疗原发性肝癌,近期疗效优于单纯TACE治疗,不良反应大多数患者均可耐受,是一种比较理想的综合治疗方法。

Abstract:Objective To compare the therapeutic effects of three dimensional conformal radiation(3-DCRT) combined with transcatheter arterial chemoembolization(TACE) and TACE for the PLC.Methods:Fifty-four patients were divided into 3-DCRT+TACE and TACE randomly.All of the patients received two cycles of TACE.Recent effect was evaluated according to WHO tumor evaluation standard and long term effect by three-year survival rate.Results:All patients finished the treatment.The total response(CR+PR) in 3-DCRT+TACE was 70.4% and in TACE was 55.6%.The overall survival rates at 1,2 and 3 years were 74.1%,44.4% and 29.6% in 3-DCRT+TACE,and 59.3%,33.3% and 18.5% in TACE group.Conclusions:3-DCRT combined with TACE has definite better therapeutic effect on PLC than TACE,and most of the patients can endure the side effect.
