骨盆骨折致坐骨神经损伤机制及预后的相关因素

目的：探讨骨盆骨折所致坐骨神经损伤的创伤机制及影响预后的相关因素。方法：分析本组骨盆骨折所致的坐骨神经损伤共53例：39例行骨盆骨折复位内固定、坐骨神经探查松解手术治疗，14例保守治疗。结果：获得随访47例，随访12个月～6年，平均2年。根据MCRR标准，手术治疗组优19例、良10例、一般4例、差1例，保守治疗组优6例、良2例、一般3例、差2例。结论：骨盆骨折致坐骨神经损伤者宜早期行骨折复位内固定，坐骨神经探查松解减压或束膜吻合，可获得较好疗效。

Mechanism and prognostic factors of the sciatic nerve injuries caused by pelvis fracture

Objective: To study the mechanism and prognostic factors of the sciatic nerve injries caused by pelvis fracture. Method:The clinical characters of 53 cases with injuries of the sciatic nerve were observed. Fourteen cases among them were treated with non-surgical methods, while the other 39 cases were operated with the open reduction, internal fixation and neurolysis .Result: All the 53 cases were followed up. The follow-up period lasted from 1 year to 6 years with an average of 2 years. According to MRCC criteria, there were 19 excellent cases, 10 good cases, 4 fair cases, 1 poor case in surgical group and 6 excellent cases, 2 good cases, 3 fair cases, 2 poor cases in the non-surgical group.Conclusion:Early operation and neurolysis can contribute to better prognosis of the sciatic nerve injuries caused by pelvis fracture.
