摘    要:目的探讨流式细胞术(FCM)在急性白血病免疫分型中的临床意义。方法利用CD45/SSC设门的四色荧光FCM检测42例急性白血病病人骨髓细胞表面标记。结果分别以cCD3c、CD79a、cMPO作为T-ALL、B-ALL、AML的系列特异性标记。30例AML中cMPO阳性率为90.0%;12例B-ALL中cCD79a阳性率为91.7%.42例患者中有交叉免疫标记的占40.5%,其中髓系白血病中常见交叉表达淋系抗原CD56、CD7、CD19、CD10;淋巴细胞白血病中常见交叉表达髓系抗原CD13、CD33等。M2出现交叉表达现象最多见,阳性率为76.9%,常见表达CD56。伴CD56、CD7交叉表达的AML预后差。结论利用流式细胞术进行免疫表型分析对急性白血病的诊断及预后判断具有重要临床意义。

The Clinical Significance of Flow Cytometer in Immunophenotyping for Acute Leukemia

Abstract:Objective To investigate the clinical significance of flow cytometer(FCM) in immunophenotyping for acute leukemia.Methods FCM with four colors and CD45/SSC gating were used to detect the antigen expression in samples of bone marrow from 42 patients with acute leukemia.Results cCD3、cCD79a、 cMPO were used as lineage specific markers respectrely for T-ALL、B-ALL and AML.MPO protein was positive in 90.0% patients of acute myeloid leukemia.cCD79a was positive in 91.7%.patients of B-ALL.more than one lineage marker were expressed in 40.5% patientsof acute leukemia.For example,CD56、CD7、CD19、CD10 were expressed in AML and CD13、CD33 were expressed in B-ALL.In addition,76.9% patients of M2 had cross-lineage antigen expression and positive rates is the highest in AML and CD56 was commonly seen.AML with CD56 and CD7 expressing had a bad prognostic.Conclusion FCM has important clinical significance in diagnoses and predicting relapse in immunophenotyping for acute leukemia.
