可视喉镜在颧弓骨折手术中经鼻气管内插管的应用

摘    要:目的:研究氧瞬得可视喉镜在颧弓骨折手术中经鼻气管内插管的应用效果。方法选取2015年1月至2016年5月在我院行手术治疗的单纯颧弓骨折患者100例,按照数字表法随机分为可视喉镜组(A组)与传统喉镜组(B组),每组各50例。对比两组患者插管时间、声门暴露时间、首次插管成功率、插管前后收缩压、心率、术后咽痛及咽部损伤情况。结果 A组患者插管时间和声门暴露时间分别为(28.16±5.62) s、(25.17±6.44) s、均短于B组的(37.26±7.89) s和(32.68±11.83) s,首次插管成功率为96.00%,高于B组的70.00%,差异均有统计学意义(P<0.05);插管后,A组收缩压、心率分别为(110.13±8.79) mmHg和(71.29±4.63)次/min,明显低于B组患者的(126.47±9.17) mmHg和(89.47±5.32)次/min,差异均有统计学意义(P<0.05);A组患者术后咽痛和咽部损伤率分别为8.00%和2.00%,均明显低于B组的34.00%和28.00%,差异均有统计学意义(P<0.05)。结论颧弓骨折手术患者在氧瞬得可视喉镜下行经鼻气管内插管可有效提高首次插管成功率,应用效果较优。

Application of video laryngoscope in nasotracheal intubation for zygomatic arch fracture surgery
Abstract:Objective To study application effect of Yanshunde (Airtraq) video laryngoscope in nasotracheal intubation for zygomatic arch fracture surgery. Methods A total of 100 patients with simple fracture of zygomatic arch, who underwent surgical treatment in our hospital from January 2015 to May 2016, which were randomly divided into video laryngoscope group (group A) and traditional laryngoscope group (group B) according to random number table, with 50 patients in each group. The intubation time, glottic exposure time, first intubation success rate, systolic blood pressure before and after intubation, heart rate, postoperative sore throat and throat injury situation were compared be-tween the two groups. Results Intubation time , glottic exposure time in group A were respectively (28.16±5.62) s and (25.17±6.44) s, which were significantly lower than those in group B of (37.26±7.89) s, (32.68±11.83) s, and the first in-tubation success rate of group A (96.00%) was significantly higher than that of group B (70.00%), with statistically sig-nificant difference (P<0.05). After intubation, the systolic blood pressure and heart rate of group A were respectively (110.13 ± 8.79) mmHg and (71.29 ± 4.63) beats/min, which were significantly lower than those of group B of (126.47 ± 9.17) mmHg and (89.47 ± 5.32) beats/min, with statistically significant difference (P<0.05). Postoperative sore throat rate and pharyngeal injury rate of group A were respectively 8.00% and 2.00%, which were significantly lower than those of group B 34.00%and 28.00%, with statistically significant differences (P<0.05). Conclusion Application of Yangshunde (Airtraq) video laryngoscope in nasal endotracheal intubation for zygomatic arch fracture surgery may effec-tively improve the first intubation success rate, with good efficacy.
