NT-proBNP、cTnI联合检测诊断儿童肺炎并心力衰竭的临床价值

目的：探讨全血氨基末端B型钠尿肽前体(NT-proBNP)和心肌肌钙蛋白I (cTnI)联合检测对儿童肺炎并心力衰竭的诊断价值。方法选择2013年1月在2014年4月中山市博爱医院诊治的174例明确诊断为肺炎并心力衰竭患儿(观察组)及同期200例健康体检人群(对照组)为研究对象，检测其NT-proBNP和cTnI的浓度。分析两组人群的血液中NT-proBNP和cTnI水平差异以及NT-proBNP和cTnI两项指标联合检测的诊断价值。结果观察组患者全血中NT-proBNP和cTnI分别为(4142.00±847.34) ng/L、(0.024±0.002)μg/L，对照组为(167.50±94.33) ng/L和(0.018±0.007)μg/L ，与对照组比较，观察组中NT-proBNP和cTnI浓度均有升高，差异均有统计学意义(P<0.05)；NT-proBNP/cTnI联合检测诊断敏感性为96.55%，均明显大于NT-proBNP的89.08%和cTnI的80.46%，差异有统计学意义(P<0.05)。结论联合检测NT-proBNP和cTnI对儿童肺炎并心力衰竭的早期筛查和诊断具有一定的临床应用价值。

Clinical value of the combined detection of NT-pro-BNP and cTnI in the diagnosis of children with pneumonia and heart failure

Objective To discuss the clinical value of the combination detection of blood N-terminal B type na-triuretic peptide precursor (NT-proBNP) and cardiac troponin I (cTnI) in the diagnosis of children with pneumonia and heart failure. Methods A total of 174 cases of patients with pneumonia and heart failure, who admitted to our hospital from January 2013 to April 2014, were selected as the experiment group. At the same time, 200 cases of healthy persons were selected as the control group. NT-proBNP and cTnI concentration of the two groups were detected and compared. The diagnostic value of the combined detection of NT-proBNP and cTnI was analyzed. Results NT-proBNP and cTnI in whole blood of the experiment group were (4 142.00±847.34) ng/L, (0.024±0.002)μg/L, respectively, which were sig-nificantly higher than those of the control group of (167.50±94.33) ng/L, (0.018±0.007)μg/L, respectively, P<0.05. The diagnostic sensitivity of NT-proBNP/cTnI combined detection was 96.55%, which was significantly higher than that of NT-proBNP (89.08%) and cTnI (80.46%), P<0.05. Conclusion The combined detection of NT-proBNP and cTnI has certain clinical application value in early screening and diagnosis of children with pneumonia and heart failure.
