紫外线消毒运行成本及灼伤事件原因分析

摘    要:目的 了解紫外线消毒的运行成本和紫外线灼伤事件的原因,为进一步高效安全运行紫外线消毒提供参考依据。方法 收集2014年1-12月温州医科大学附属第一医院所有紫外线消毒区域(包括治疗室、处置室、输液室、血透室、分娩室、细胞分离室等)紫外线消毒的运行情况和紫外线灼伤事件发生情况,分析紫外线消毒的流动成本和固定成本以及紫外线灼伤事件发生的主要原因。结果 温州医科大学附属第一医院2014年紫外线消毒运行成本中的流动成本约为1 653 715元,固定成本约为88 872元,流动成本远大于固定成本;流动成本中,紫外线强度检测人工成本约为106 378元,紫外线每日消毒操作人工成本约为1 534 910元,紫外线灯管额外安装与维修费约为10 000元,电费约为2 427元;固定成本中,紫外线设备成本约为78 292元,紫外线设备安装成本约为10 580元;2014年共发生紫外线灼伤事件2起,主要原因为紫外线灯误开与照明用灯同时使用未发现和新护工的意外操作。结论 紫外线消毒运行中人力成本支出较高,应进行量化精准监控,优化管理流程,控制成本,并杜绝人为暴露灼伤,确保紫外线消毒的有效性和安全性。

Abstract:Objective To examine the running cost of ultraviolet (UV)equipment for air disinfection and the cause of burn injuries induced by UV disinfection in hospitals for providing evidences to effective and safe operation of UV disinfection.Methods The Data on operative status of UV equipments for air disinfection in medical settings (including rooms for therapy,treatment,transfusion,hemodialysis,and delivery and cell separation chamber)and burn events related to UV air disinfection throughout 2014 in a general hospital in Wenzhou city were collected and analyzed.Results In 2014 the total current cost for the operation of UV air disinfection in the hospital was 1 653 715 (RMB yuan) and much higher than the total fixed cost (88 872);the current cost consisted of the labor costs for UV intensity detection (106 378) and daily disinfection operation (1 534 910),the cost for replacement and repair of ultraviolet lamp (10 000),and the cost for electricity (2 427);the fixed cost consisted of the cost for purchase (78 292) and installation (10 580) of UV equipments.Totally two radiation burns related to UV air disinfection were reported during the year and both the burns were caused by misoperation of UV disinfection equipment.Conclusion The manpower cost is relatively higher for hospital UV air disinfection and cost control and radiation burn prevention should be concerned for the efficient and safe operation of UV equipment for air disinfection at medical settings.
