无功能性恶性胰岛细胞瘤10例报道并文献复习

目的 :探讨恶性胰岛细胞瘤的临床特点和治疗方法。方法 :本组 10例全部经手术治疗。术后化疗 4例 ,所用药物包括表阿霉素 (E ADM)、丝裂霉素 (MMC)、5 -氟尿嘧啶 (5 FU)、顺铂 (DDP)。其中3例因肝转移行肝动脉插管化疗及碘化油栓塞 ;2例行局部放疗 ,1例瘤床照射 5 0Gy ,另 1例胰头部复发照射 4 5Gy。结果 :目前仍生存 5例 ,最长已生存 2 6年 ;死亡 5例 ,存活时间分别为 2年、2年、2年 3个月、3年、13年。结论 :手术是治疗恶性胰岛细胞瘤的首选治疗方法 ,化疗具有一定疗效。肝转移病例经导管肝动脉栓塞或栓塞加化疗是一种有效的治疗方法。放射治疗的价值有待进一步明确

Nonfunctional Malignant Islet Cell Tumors:10 Cases Report

Objective To investigate the characteristics of malignant islet cell tumors.Methods Ten patients underwent surgery,4 of them received chemotherapy of E ADM,MMC,5 FU,DDP.Three cases treated with transcatheter arterial embolization and infusion of anti tumor drugs because of live metastases.Local irradiation was used in 2 patients,50 Gy for tumor bed in one patient and 45 Gy for recurrent lesion in another.Results Five patients are alive,the longest survival is 26 years after surgery.Five died 2 years,2 years,2 years and 3 months,3 years,and 13 years respectively after first operation.Conclusions Surgery is first option for malignant islet cell tumor.Chemotherapy is profitable to these tumors.Transcatheter arterial embolization and/or infusion of anti tumor drugs is effective modality for liver metastasis.The effect of radiotherapy for malignant nonfunctional islet cell tumor is not clear.
